CMAthletic Uniform Agreement
I agree to replace the uniform if it is damaged; this means lost, torn, stained, or anything that changes the appearance from when I received it.*

CMAthletic Tournament Agreement
I agree to pay for all tournament fees associated with my team whether or not I am able to attend.*

CMAthletics Concession Agreement
I, or an approved replacement, will work my shifts in the concession stand during the season. If I am unable to do so, I will find a replacement or pay a $25 buyout per missed shift.

CMAthletics Sports Release & Authorization
I agree to hold Christian Ministries Academy and its agents harmless of any liabilities resulting from injuries or loss of property sustained by me/my child during any athletic function. I give consent for my child to receive medical treatment by onsite medical personnel, a registered nurse or licensed physician when deemed necessary by school officials. The above mentioned student(s) is physically fit, mentally stable and prepared to participate in CMAthletic events.*
