CNL FAMILY CAMP

• Please register as a group with your preferred roommates, where applicable.
• For group registrations, payment will be handled by your designated Point-of-Contact.
• Children aged 3 and under are not required to submit individual details. They are Free of Charge. 

• 请与您的室友以小组形式一同报名。
• 团体报名将由您指定的主要联系人统一处理付款。
• 3岁及以下的儿童无需提交个人资料，且费用全免。

Support & Subsidies (资助与津贴): Eligibility Note

• The S$40 subsidy is provided for CNL church attendees requiring financial assistance.
  
  This subsidy is not applicable for Student or Child camp fees, as those rates are already subsidized.

• S$40 个人津贴仅适用于需要财务资助的 CNL 会友。
  
  此津贴不适用于学生或儿童报名费，因为该费用已包含津贴。

• Requests submitted are subject to availability and cannot be guaranteed.
• Payment is not requried after filling out this form. Please look out for camp committee's email for payment amount and instructions.

• 所提交的特殊要求将视供应情况而定，无法保证一定能安排。

• By submitting this form, I hereby consent to the collection, use, disclosure and retention of my personal data for the purpose of the church camp registration, and in accordance with privacy policy of Centre of New Life (CNL).
• By providing my contact details, I agree that CNL and the church camp committee may contact me on any matters relating to CNL and the camp.
• I further acknowledge that official photos and videos will be taken by CNL during the church camp, and that these may be used by CNL for internal and external publicity purposes.
