    Please provide the name and email address of a teacher who can serve as a reference. Art teachers are preferred. The teacher will be contacted to provide a reference for your scholarship application. Be sure to confirm that the teacher has received and submitted the reference form.     Video samples must be uploaded to a video hosting service, such as YouTube or Vimeo.

    Remember: you are limited to a total of 6 submissions. Required Image Uploads should include screen shots of the work. Use the Image Medium field to clearly indicate that the work is time based, and the Dimensions field should include the duration (min,sec).

    Video samples are limited to 5 minutes total, and no more than 3 links.
