  • Extension of an Incomplete

    Students who received a grade of Incomplete in a past term, and wish to extend the deadline for completion of the coursework, must submit a petition.

  • Students MUST submit a completed Incomplete Grade Agreement signed by both the Instructor and Student.  This form can be submitted to your academic advisor and/or the Arts and Sciences Student Services Center.

  • Foreign Language Requirement Substitution or Exemption 

    The Arts and Humanities programs of the College of Arts and Sciences have an additional Foreign Language Requirement beyond the state entrance requirement. Students who wish to modify this requirement must submit a petition. NOTE: Heritage and native speakers should not file this petition and should instead contact the Department of World Languages, Literatures, and Cultures for assessment.
  • Upper Division Credit Waiver

    The College of Arts and Sciences has an upper division credit requirement. Students who first enrolled at CSU prior to Fall 2024 must complete a minimum of 42 upper-division (300/400-level) credit hours. Students who first enrolled in courses in Fall 2024 or after must complete a minimum of 36 upper-division (300/400-level) credit hours. NOTE: Advisors are authorized to allow students to graduate with a one fewer upper division credit hours (a total of 41 upper-division hours for those who first enrolled prior to Fall 2024, and a total of 35 upper division hours for those who first enrolled in Fall 2024 or after). This can be done directly with your college or program academic advisor without petitioning.
  • Retroactive Registration

    Students wishing to retroactively add a course or courses to their schedule, once the official registration period in CampusNet has expired and the enrollment form with instructor signature(s) is no longer valid, must submit a petition.
  • If approved, payment in full (which may include a tuition surcharge or late registration fee) will be due immediately. Upon proof of payment, the course(s) will be added to the student's schedule. If the course(s) listed below are from a semester which has already concluded, it is the responsibility of the student to contact the instructor(s) and arrange for the submission of a final grade(s).  

  • Reinstatement

    Students who were previously dropped from their course(s) due to nonpayment who now wish to have the course(s) restored to their academic transcripts must submit a petition.
  • Reinstatement applies to the entire schedule at the time of course cancellation; students cannot select individual courses to be reinstated.  For any course(s) in which the instructor is unable to provide a final grade, a failing grade will be assigned.  If the reinstatement is approved, payment in full (which may include a tuition surcharge) will be due immediately. Upon proof of payment, the course(s) will be returned to the student's transcript. It is the responsibility of the student to then contact the instructor(s) and arrange the submission of a final grade(s). 

    Barring extreme circumstances, a petition for reinstatement will be entertained only once during a student's time at CSU.

  • Credit Overload

    Students must submit a Credit Overload request in order to register for more than 19 credit hours in any single term. Students in the Mandel Honors College must seek approval through the Honors College and may NOT utilize this form.  
  • Following the submission of a narrative (on the next page), students will be contacted regarding an appointment with their advisor.  This petition requires that a student and their advisor discuss this request. 

     

  • Late Withdrawal

    Students wishing to withdraw from a course or courses once the official drop/withdraw period in CampusNet has expired must submit a petition. NOTE: Students wishing to withdraw should consider the impact that withdrawal may have on their ACADEMIC STANDING as well as their FINANCIAL AID award (generally, students must complete at least 67% of their courses each term to remain compliant with both academic standing and financial aid policies.
  • Poor academic performance in a course(s) is not grounds for late withdrawal. The committee is reluctant to grant selective withdrawals, especially when it is clear that the withdrawal is only requested for courses with poor grades. Late withdrawal circumstances typically refer to unforeseen circumstances, with a clear rationale as to why you were unable to withdraw prior to the University-established deadline.

    Successful late withdrawal petitions often include relevant medical, legal, or other documentation, if you are willing and able to provide such materials.   Any supplemental materials should be submitted directly to the Arts and Sciences Student Services Center or your academic advisor.

  • Following the submission of a narrative (on the next page), students will be contacted regarding an appointment with their advisor.  This petition requires that a student and their advisor discuss this request, and that the advisor submit a statement to accompany the petition. 

    If you have documentation (medical, legal, etc) you would like to add, you can provide it directly to your advisor.

  • Readmission

    Students who were previously dismissed/suspended from Cleveland State and wish to return must submit a petition.
  • Additional Statement on Criminal Offenses

    Cleveland State University believes students are more than their record. We are dedicated to providing an inclusive pathway to higher education and successful re-entry for the formerly incarcerated. The university will fairly consider all applicants regardless of previous charges or convictions. Responses to these questions are kept confidential.

  • Following the submission of a narrative (on the next page), students will be contacted regarding an appointment with their advisor.  This petition requires that a student and their advisor discuss this request, and that the advisor submit a statement to accompany the petition. 

    If you also need to submit transcripts from another institution, you can provide those directly to your advisor. Unofficial and "in progress" transcripts can be used for the purposes of the petition.  If readmission is granted, official/final transcripts will be required.
