  • Note to examiners: Only applicants who were promoted on the SAME date can be processed on one form. If you have multiple testing dates, you have to submit seperate forms. 

    Applicants emails (personal or parent/guardian) need to be filled in below. DO NOT use the club or instructors emails. ITF does not allow duplicates, and an email is needed in order for members to access their personal information if needed.

  • You must include a photo for every person on this application form. If there are any photos missing, that person will be removed from the application and the examiner will have to resubmit it when the photo is obtained.

    Photos must be under 200kb and be in jpeg or jpg format.
    Photos should be of the head only (like a passport photo), and not below the shoulders.

  • Please select the amount of each certificate you are submitting. 

    *Reminders:

    All 4th Dan and higher MUST have the current year plaque paid for in order to process their application.  Please select this option at the bottom if you have not yet paid the plaque fee for the current year.  It is the the responsibility of the instructors to collect this from their students and submit to CTFI.  Applications will not be processed until the plaque fee is paid*

    *All 4th Dan and higher MUST complete an IIC course before they are eligible to be promoted. Please ensure with your student all criteria has been completed before applying*
