    Use this form to request gift cards for volunteer/staff appreciation or prizes for games. All gift card distribution is centralized through the CTK Finance Team.

  • Please talk to your supervisor or the finance department for approval before proceeding. Thanks!

    All requests must include the person's Full Name & Gift Card Type:

    • Jimmy John's ($15)
    • Amazon ($25)
