Please read before submitting this form:
1. Casual Wear Introduction: No extra photo is needed for casual wear; the competition is on stage only. Write your onstage introduction in the third person on this form as follows:
Example: Susie Jones is wearing a flowered sundress paired with a matching hat and shoes. The perfect outfit for a Sunday tea or an afternoon with friends in the park.
2. On Stage Talent introduction should include any Equipment needed, an example is: a Handheld mic, Mic stand, etc. Start your intro with the type of talent: Vocal, Dance, Monologue, etc.
Example: Susie Jones is performing a jazz dance to Happy Birthday; she has studied dance for 5 years at the Dance Academy in Bangor. YOU MUST ATTEND THE SOUND CHECK.
Please note: we do not have a piano, props, or mats; you will need to bring a keyboard if you are playing the piano for talent, props or mats for tumbling, or you can also submit a piano, tap, or mat type talent as a non-performance talent. The stage is approximately 32’x16’ for any tumbling or dancing and is carpeted. The runway is not carpeted if you are tap dancing.
3. Free competitions require additional submissions; see the Contestant Handbook for details.
4. Bring your Role Model book to States with you; you do not need to mail it.
5. Upload Publishers Award, Non-performance talent, and spokesmodel competition material on this form.
6. To participate in Miss Maine Ambassador, you need to raise a minimum of $900. See details in the handbook as rules and guidelines have changed.
7. We do not accept PayPal. You are responsible for the remaining balance if you pay for goods and services. To Venmo Crossroads: @deb-landry. We suggest Venmo as it is fast, easy, with no charges, and the best way to shop at States!
8. All registration forms must be completed online. We will invoice you if you prefer to pay by credit card. Only mail checks.
9. Upload all your photos (high res). We are not responsible for low res photos and will print what is sent.

11. To participate in the Platform ad page optional requires additional photos and material.
12. Paperwork must be filed online in three forms:
1. Registration and informational form (24 hours after winning your local pageant)
2. Resume and photo (by May 1)
3. Checklist (this form) by May 15. Nothing needs to be mailed except checks. All paperwork is now online.

Release: By signing this electronic form, I certify that I identify as female and am a U.S. citizen of Maine or a Canadian citizen. I agree to accept the terms as explained in the delegate manual and understand the policies set forth by Crossroads, including the State and International Code of Conduct Policies. This application is a confirmation and a commitment that I will compete in the state pageant, and I assume all responsibility for p program fees, tickets, and required wardrobe. I understand that if I am unable to attend the pageant I have signed up for after I have registered, paid, or won a secondary title and do not attend or participate in the pageant the following year for any reason without a physician’s note; I must forfeit my local or state title, must return the crown and sash and will pay a $350 withdrawal fee. I understand that all monies paid or donated to Crossroads are non-refundable, made willingly, and returned checks incur a cost of $40.
If I withdraw, I will not receive any prizes or scholarships won in the local, state, regional, or national pageant for that year and must return what I have received. Withdrawal includes secondary titles awarded to the first runner-ups, scholarships, and awards in all divisions or any award promised after the crowning. Delegates and their parents or guardians hereby release and forever discharge Crossroads, Maine Academic Scholarship Program, Crown CARES, American Women of Service, Young American Women of Service, or any program Crossroads contracts within pageantry and the international, regional, local, or state pageants in which I competed, including its directors, employees, volunteers and staff from all claims and demands arising from damages or liabilities due to theft, accident, loss or injury resulting from participation in the programs mentioned above. I understand that it is my choice to participate in this program.
Any delegate competing to qualify in Young American Women of Service or a Miss division of our locals must certify that they have never been married, have children, or are pregnant. All delegates and their parents/guardians further covenant and agree not to sue the organization, its directors individually, its employees, volunteers, or staff for any claim for damages or liability, including and not limited to not winning your title. I agree that I am of exemplary character and will abide by all of the rules and regulations set forth by Crossroads, which include appearances and fees to compete.
All pageants and Crown CARES events serve as a fundraiser for Crossroads 501c3 organization and support their general mission. Fees are non-refundable and used at the discretion of Crossroads. Extra fees or fees paid in advance must be in full if you drop out of the program without paying the agreed amount. By signing below, I am certifying that I have read and agreed to all rules listed (local, state, regional, and international) published on the organization’s websites, delegate handbooks, and applications and that I am 18 or older or my parent or guardian has signed on my behalf. Crossroads reserves the right to choose and grant awards, scholarships, and their participation in any external pageant systems if any. I have read the instructions and rules and understand that this document is legally binding by signing your name.
