Because we work with children it is important that use due diligence in screening individuals for employment and volunteer service. In connection with your employment and/or volunteer service with Camp David International Inc / YWAM Cross Wind (the “Company”), notice is hereby given that a consumer report and/or investigative consumer report may be obtained from a consumer reporting agency for employment purposes. These reports may contain information about your character, general reputation, personal characteristics and mode of living, whichever are applicable. They may involve personal interviews with sources such as your neighbors, friends or associates. The reports may also contain information about you relating to your criminal history, driving and/or motor vehicle records, education or employment history, or other background checks.

You have the right, upon written request made within a reasonable time after the receipt of this notice, to request disclosure of the nature and scope of any investigative consumer report prepared by contacting the Company and Protect My Ministry 14499 N. I am committed to serving the Lord at Camp David with all my heart and energy for the full time of service. This includes Sunday evening through Friday afternoon of my week(s) of service. The only thing that will keep me from this commitment would be a serious health issue or family emergency.I understand that while attending Camp David I may be involved in various activities including, and not limited to swimming, fishing, horse rides, field games, crazy fun and other traditional camp activities.I understand the mission for Camp David is to provide a Christian Camp where one can experience love, find hope, and grow in a relationship with Jesus Christ.I accept the risks involved in camp activities and do hereby release Camp David International and Camp David Kansas, and all staff and volunteers, from any and all claims for injuries or death sustained by me or my family, whether such injury occurs on or off the camp property.I give permission to Camp David to use camp pictures or videos including myself in promotional and sponsorship material.I understand that Camp David is not responsible for lost clothing or personal property while I am at camp.
