"I accept the risks involved in camp activities and do hereby release Camp David Kansas, and all staff and volunteers, from any and all claims for bodily and personal injuries or death sustained by my child, whether such injury occurs on or off the camp property. I give permission for Camp David Kansas to secure emergency medical and surgical treatment, and routine, non-surgical medical care for my child while at camp. I understand that should my child become ill I may need to make arrangements to pick them up."

Camp David Kansas is located in Hesston, KS. If necessary, Camp David can provide transportation for your child from pick-up points in several cities around Kansas.
Note: Camp starts at 11:30am on Monday and ends at 1:00pm on Friday. You will receive a letter confirming your transportation plan one week prior to your child's week of camp!

While attending Camp David Kansas my child may be involved in various activities including, and not limited to swimming, horse rides, obstacle course, and other traditional camp activities.The mission of Camp David Kansas/International is to provide a Christian Camp where one can experience love, find hope, and grow in a relationship with Jesus Christ.I am granting permission for my child to attend camp and participate fully in all camp activities.My son/daughter/foster child must obey the rules of Camp David Kansas, and I have instructed them to do so.Should my child become very disruptive, sick or injured to the point they can no longer participate, I will need to make arrangements to pick them up from Camp David Kansas.I also give permission to Camp David Kansas to use camp pictures of my child in promotional and sponsorship material.When participating in this Angel Tree associated camp (Camp David Kansas), I understand that my child may be photographed and videotaped, and that my child’s first name and biographical information, and/or comments and quotes may be obtained (statements); and that such images and statements (materials) may be used by Prison Fellowship in promotional materials, news releases, websites, and other published formats. I hereby release Prison Fellowship and anyone working on their behalf from any and all liability, claims and causes of action that I might have arising out of the use of such materials, to include rights of publicity and privacy. The materials will be the sole property of Prison Fellowship and Camp David Kansas/International.
