    Outpatient Care: Outpatient care is the care that is entirely completed on the day of your animal's visit. Outpatients that are not hospitalized must have balances paid in full at the time services are rendered.

    Acceptable means of Payment include: Cash Visa MasterCard Discover or Care Credit Financial Services

    Personal Checks are NOT accepted as a form of payment.**

    Inpatient Care: If your animal requires hospitalization, you will receive an estimate for at least the first 24 hours of care. This is only a preliminary estimate and may change if additional diagnostics or care is needed. Estimates are an approximation only and the final bill may be greater or less than the amount specified. A deposit of 75% of the estimate must be collected at the patient's time of admission. A payment of 100% of the total bill must be paid at the time the patient is discharged. NO ADDITIONAL SERVICES may be charged to any accounts with an outstanding balance or past due amount. Instead, all fees for such services or products MUST be paid (cash or major credit cards - No Care Credit) at the time of service.

    APPOINTMENT CANCELLATION, RESCHEDULING & NO-SHOW POLICY: Canterbury Animal Hospital's doctors and staff strive to provide high quality veterinary care and client services. Missed appointments without adequate notice create scheduling issues. In order for us to best accomodate our clients and patients, we require any cancellations/rescheduling of appointments/rechecks need to occur 24 hours prior to the scheduled time or a charge of $55.00 will be applied to the account. Any No- Shows will incur a charge of $67.50. Any cancellations/rescheduling of surgical procedures needs to occur 48 hours prior to the procedure date or a charge of $75.00 will be applied to the account. ** Future appointments or procedures will not be scheduled until the outstanding balance on the account is paid in full.  

    Surgical deposit of $75 is collected at the time of surgical scheduling. The deposit will go towards the procedure unless you No Show/Cancel/Reschedule within 48 hours to the scheduled surgical date.

    I certify that I am the owner or duly authorized agent for the owner and as signer, have the authority to authorize treatment for the animals listed under Canterbury Animal Hospitals care. I certify that I have read, understand and agree to this payment policy.
