  • Consent to Participate

    I am the parent or legal guardian of the minor named above. I give permission for the minor to participate as a volunteer with CARE Complex during the 2026 Winter Wonderland Event or Tree Lighting Event and related preparation activities.

    I understand that volunteer activities may include sorting and organizing clothing, toys, winter supplies, and other donated items; assisting with setup and cleanup; greeting attendees; distributing items; and performing other age-appropriate volunteer duties.

    I understand that CARE Complex staff or volunteer leads may reassign or restrict duties when necessary for the minor’s safety or the safe operation of the event.

  • Thank you for volunteering at CARE Complex’s Winter Wonderland 2026 event. To help ensure a safe, respectful, and positive experience for everyone involved, all volunteers are required to complete this Liability and Media Waiver.

    Media Consent
    I grant CARE Complex and its event partners permission to photograph, film, and/or record me while I am volunteering at the 2026 Winter Wonderland Event or participating in related CARE Complex activities.

    I understand that photographs, video footage, and audio recordings may be used for:

    • Social media and website content
    • Printed promotional or educational materials
    • Reports provided to donors, sponsors, or grant funders
    • Public presentations and educational campaigns

    I understand that this content may be edited, reproduced, published, or combined with other materials. I waive any right to compensation, ownership, or additional approval regarding its use now or in the future.

    Voluntary Participation and Assumption of Risk
    I understand that my participation as a volunteer is voluntary. I acknowledge that volunteering may involve physical activity, lifting, carrying, standing for extended periods, event setup and breakdown, interacting with members of the public, and other event-related responsibilities.

    I voluntarily accept and assume all risks associated with my participation, whether known or unknown.

    Group Participation
    If I have registered as a group leader, I confirm that I am signing this waiver on behalf of myself and all members of my volunteer group. I confirm that every group member is at least 18 years of age and has been informed of and agreed to the terms of this waiver. I accept responsibility for ensuring that all members of my group follow CARE Complex’s instructions, safety procedures, event guidelines, and standards of conduct. To the fullest extent permitted by law, I also accept responsibility for the actions, conduct, and participation of all members of my group during volunteer activities.

    Release of Liability
    To the fullest extent permitted by law, I release and hold harmless CARE Complex, Civic Center – Goodman Outdoor Plaza, event partners, sponsors, staff members, board members, contractors, volunteers, representatives, and affiliates from any claims, demands, damages, losses, or liabilities arising from or related to my volunteer participation, except where prohibited by law.

    Medical Authorization
    In the event of an injury or medical emergency, I authorize CARE Complex representatives to contact emergency medical services and seek appropriate medical care on my behalf.

    I understand that CARE Complex cannot guarantee the availability or outcome of medical treatment, and I accept responsibility for any medical expenses resulting from such treatment.

    Confidentiality and Privacy
    I agree to respect the privacy, dignity, and confidentiality of event attendees, volunteers, staff members, and community partners.

    I will not photograph, record, share, or disclose personal information about anyone participating in the event unless I have received prior authorization from CARE Complex.

    I understand that my contact information may be securely stored in CARE Complex’s customer relationship management system and used for volunteer coordination, event communication, and related organizational purposes.

    Volunteer Conduct
    I agree to: Follow all instructions, safety procedures, and event guidelines
    Perform only the duties assigned to me by CARE Complex staff
    Conduct myself respectfully and professionally
    Treat all attendees, volunteers, staff members, and partners with dignity
    Immediately report any safety concerns, injuries, or inappropriate conduct to a CARE Complex staff member
    Refrain from using alcohol, illegal substances, or engaging in disruptive behavior while volunteering
    I understand that CARE Complex may end my volunteer participation at any time if I do not follow event rules, safety procedures, or standards of conduct.

    General Agreement
    This waiver is valid for the 2026 Winter Wonderland Event and any directly related preparation or event activities for which I volunteer.

    By signing below, I confirm that I am at least 16 years of age, have read and understood this agreement, and voluntarily accept all terms stated above.
