LIABILITY, POLICIES & AGREEMENT
Medical Disclaimer & Assumption of Risk
I acknowledge that participation in dance classes, rehearsals, workshops, and performances with Carisma Dance Academy involves physical activity and inherent risks, including injury. I confirm that I am physically able to participate and have no medical condition that would limit my participation. I accept full responsibility for my health and well-being and agree to consult a physician if I have any concerns. I voluntarily assume all risks associated with participation, whether known or unknown.
Release of Liability & Waiver
In consideration of my participation, I hereby release and hold harmless Image Concepts LLC and its division, Carisma Dance Academy, including its directors, instructors, staff, contractors, and affiliates, from any and all claims or liabilities arising from my participation, including personal injury, illness, or property damage, to the fullest extent permitted by California law.
Photo, Video & Likeness Release
I grant Image Concepts LLC and Carisma Dance Academy the unrestricted right to photograph, record, and use my image, likeness, voice, and performance in photo, video, and digital media for promotional, educational, archival, and marketing purposes, in perpetuity, without compensation. I waive any right to inspect or approve the final materials.

All personal information is kept confidential and used only to support your growth and goals.
Punctuality & Courtesy
Please arrive on time out of respect for your instructor and other scheduled sessions. If you are running late, call or text us. We will do our best to accommodate you; however, arrivals more than 10 minutes late may result in a shortened session.
Health & Safety
Carisma Dance Academy follows CDC and local health guidelines. If you are feeling unwell, please stay home. Hand sanitizer is encouraged, and masks are always welcome.
Cancellations
Cancellations must be made by phone or email at least 48 hours in advance (24 hours for online sessions). Cancellations made after this window are non-refundable, as your instructor has reserved and prepared this time for you.
Instructor Changes
If an instructor needs to cancel or reschedule, you will be contacted promptly by phone or email. Please ensure your contact information is accurate.
Final Agreement
By submitting this form, I confirm that I have read, understood, and voluntarily agree to all policies and releases stated above.
