Order your USOMC medals and certificates. Please note the shipping and order deadlines below.
This form is for prizewinners who did not receive their medals and certificates during the Prizewinners concerts February 20 and February 21. We can ship up to 4 medals+certificates for a contestant per shipment ($40) to the same address. If you have to ship additional, please fill this form out again, and add a note. Shipment may take 3-4 weeks. If you have any problems, please email info@usomc.org. This form will close March 22, 2025.
