1. Submission of this form does not guarantee acceptance as a vendor for the event. The event organizers reserve the right to accept or reject any vendor application. 2. Vendors are responsible for obtaining any necessary permits/licenses required for their products/services 3. Booth allocation, set-up details, maps, etcwill be provided the week prior to the event. Vendors are required to be set-up throughout the duration of the event. No early break down is permitted. 4. Vendors are expected to adhere to all event rules and regulations provided by the organizers. 5.The event organizers will not be held responsible for any loss or damage to vendor merchandise or equipment. 6. No refunds will be given unless the vendor is rejected by SMPOA. 7. By submitting this form, you agree to abide by the terms and conditions set forth by the event organizers.
