The above-described animal is being released to me. I understand and agree to the following conditions of this rehoming
Please initial the following:
I will provide adequate food, water, shelter, space, care, transportation, training, grooming, and veterinary treatment for this animal.

I will comply with all state/city animal license and rabies requirements and will maintain the animal in a healthy manner.

I will not allow this animal to run at large in violation of local animal control ordinances.
*
I will get this animal altered within 30 days of the agreement signed. Large breeds can wait until 1 year of age. If this step is not completed and proof isn't sent to prior owner. Prior owner has rights to take the animal back

I will not chain, tether, or cage the animal (crate training is permitted)
*
l will not have any inhumane procedures or acts done to the animal.

I accept the animal in its current condition and with all its faults, conditions,or any other issues, if any and I acknowledge the releasing agent has no further responsibility to me or any other party for the conditions, faults etc.

I will administer monthly heartworm prevention to this animal to ensure the animal stays heartworm free.

I agree not to have ears cropped, tails docked (unless medically necessary), or declawed.

I understand that no further guarantee of any kind can be given regarding the condition/behavior of the animal I am adopting except as stated in health records. I understand all medical bills incurred will be my sole responsibility.

I am certain I am committed to this animal for its entire life. If for any reason I cannot keep the animal at any point in time I am to notify prior owner immediately to return animal.

I understand that I am responsible for any damages and or injuries to person or property caused by this animal.

I will not have the animal euthanized for any reason other than an end-stage injury or illness where the animal is suffering, nor will I surrender the animal to Animal Control or an individual.

I acknowledge that this is a binding contract,and by accepting this agreement, I hereby release the agent and/or representatives from any responsibility for any injuries, illness, or damages/costs incurred as a result of the adoption of this animal.

If I am unable to keep the animal, I must notify prior owner

If the animal gets loose, runs away, is lost/stolen, or passes away I must notify prior owner
*
In the event the terms of this contract are not satisfied by me, or if the releasing agent is dissatisfied at any time with the care and treatment which the animal receives while in my custody, I agree that prior owner has the right to take custody of animal
