I hereby grant permission for myself, my child and/or family members to be photographed by the Coast Guard Foundation for any legitimate purpose without payment or compensation. I hereby grant the Coast Guard Foundation permission to use any photos I submit of myself, my child and/or family members for any legitimate purpose without payment or compensation. I hereby consent to the participation in interviews, use of quotes and the taking of photographs, movies or video tapes. I grant to the Coast Guard Foundation the right to edit, use and reuse the products/content for nonprofit purposes including in print, or on the internet, and all other forms of media. I hereby release the Coast Guard Foundation and its agents and employees from all claims, demands and liabilities whatsoever in connection with the above.
I understand that I can revoke permission for said use any time by emailing info@coastguardfoundation.org and requesting the removal of an image of me, my child and/or family members.
