PLEASE READ
In the interest of your pet(s) and the others staying with us we have certain requirements which are as follows:
- If your pet has been sneezing or coughing it must be examined by one of our Veterinarians to determine if he/she may board at our facility. This is to prevent spread of infectious disease.
- All animals are required to have current vaccinations. For dogs, this means a DHLPPC, Rabies and Bordetella.
- For cats, this means a FVRCP, Rabies and Feline Leukemia. If your pet(s) are not current on vaccines, we must vaccinate them at your expense. Proof of vaccines must be provided for animals that we do not have on record.
- Should fleas or ticks be present on your pet upon arrival, they will be treated at your expense.
- Reasonable precaution will be used against injury, escape, or death of your pet(s).
- The hospital and staff will not be held liable for problems that develop provided reasonable care and precautions are followed.
- I understand that anyproblem(s) that develop with my pet will be treated as deemed best by the staff veterinarian(s) and I assume full responsibility for the treatment expense involved. I further understand that even though my pet(s) are fully vaccinated there is always a chance of infections being transmitted to them and I accept the liability of any necessary treatment of such infections.
- Reservations for boarding that are not kept and not cancelled at LEAST 24 (business) hours in advance are charged at fifty percent of the current rate for all the nights reserved. This applies to early check out also. You are responsible for obtaining a cancellation number if you cancel or modify your boarding reservation. We cannot accept cancellations left on voice mail after hours.
- If for any reason you are not able to pick up your pet(s) on the date you specify you must notify us of this within atleast 24 hours. Payment for accrued charges and balance of stay must be made at this time and will be charged to the credit card on file unless you provide a different means of payment. Every possible accommodation will be made to board your pet through the new date you give us or if that is not possible, we will assist you in arranging for a transfer to a different boarding facility.
- If you do not pick up your pet(s) and do not notify us of your change of plans, your pet(s) will be considered abandoned and we will follow the guidelines set forth in California Code 1834.5 regarding the disposition of abandoned pets which is printed below. This action shall in no way relieve you from the responsibility of charges incurred; including charges during the waiting period set forth in the afore mentioned code. You will also be held responsible for any fees incurred in the collection of these charges. 1834.5 Abandoned animal; disposition; notice: Notwithstanding any other provision of law, whenever any animal is delivered to any veterinarian, dog kennel, cat kennel,pet-grooming parlor, animal hospital, or any other animal care facility pursuant to any written or oral agreement entered into after the effective date of this section, and the owner of such animal does not pick up the animal within 14 calendar days after the day the animal was due to be picked up, the animal shall be deemed to be abandoned. The person into whose custody the animal was placed for care shall first try for a period of not less than 10 days to find a new owner for the animal, and, if unable to place the animal with a new owner, shall thereafter humanely destroy the animal so abandoned. If an animal so abandoned was left with a veterinarian or with a facility which has a veterinarian, and a new owner cannot be found pursuant to this section, such veterinarian shall humanely destroy the animal.
