Service Name: “Patient Connect Online Services”- chat-based care services for subscribers of the online platforms operated by Patient Connect LLC, Miami. Services: Patient Connect will provide chat-based care services including mental health counselling, medical and specialist care, fitness, wellness, dietitian and nutritional support, pharmacy, dental, physiotherapy, vet, optical, financial, legal, and other support services to subscribers who engage these services. Eligibility: Care providers must be registered by the governing body of the profession they are representing under the laws and regulations as prescribed and governed by the laws of Barbados and other territories where registered. Where no such body exists, care providers will be appropriately vetted by Patient Connect. Care providers must advise Patient Connect if they are currently before the Courts of Barbados or any other jurisdiction or if they have a pending negligence or malpractice matter before their governing body. Confidentiality: All communications between the subscriber and care provider must be confidential. Protected Health Information (PHI) is protected under HIPAA regulations signed by Patient Connect through its BAA with Amazon Web Services (AWS). Information about the employee's virtual or in-clinic sessions must not be shared with any third parties or companies or anyone else without the subscriber’s written consent or when being referred to another health professional. Limitations of Services: Patient Connect’s platforms are not a substitute for urgent or emergency medical or psychiatric treatment. Care providers may refer the subscriber to other medical professionals or healthcare providers if necessary. This limitation will be clearly stated on the Patient Connect app's and website. Availability: Patient Connect’s Live Chat features will be available 24 hours a day and seven days a week, including public holidays, and is based on the care provider’s availability. Subscribers can access the chat-based service via a QR, weblink, email or WhatsApp. The articles and blogs are also available 24 hours every day. Onboarding and scheduling of care providers will be discussed by the Patient Connect team via webinars and direct engagement. Usage: Care providers are not to use the Patient Connect chat-based services for their personal client or patients. The chat-based services are only for use by Patient Connect’s business partners. If you require electronic health records, patient portal, Telehealth and remote patient monitoring (RPM) services for your private use, you may contact a duly authorised Patient Connect agent. Cost: There is no cost to the care provider for the use of the chat-based care services. Remote Patient Monitoring (RPM) services and fees can be requested by contacting a Patient Connect agent. Fees: Patient Connect LLC expects that each care provider will charge competitive market prices for in-person visits and virtual visits. For in-person visits, the subscriber directly pays the care provider. For virtual visits, if the care provider already has online banking or online credit card processing services, they may provide this to the subscriber. If they do not, Patient Connect can collect the money on the care providers’ behalf and make the necessary remittance within seven working days. Please note that online credit card processing services charge interest rates and fees so the remittance amount may be less than the cost charged to the subscriber. Patient Connect charges a US$1.00 processing fee per video call for those made on the main Telehealth/Patient Portal but does not charge any fees for those made on the chat-based voice or video services. Care providers will be wholly responsible for any insurance claims made. Upgrades and servicing: Patient Connect LLC will provide at least seven (7) days notice to all care providers when planned upgrades and/or servicing of the app and website are required. However, there may be times when immediate upgrades are needed and every effort will be made to notify care providers and subscribers via various media such as on the website, WhatsApp or via email. Termination: The various services may be terminated by Patient Connect LLC, care providers or any engaged company with one month’s notice. Indemnification: Patient Connect LLC, Amazon Web Services and our partners are indemnified against any claims or damages arising from the care provider's negligence or breach of professional codes and conduct of standard practice by virtue of this agreement between Patient Connect LLC and the care providers.
