2026-27 Acknowledgement of Tryout Policies/Process
Prior to arriving at your player's check-in for tryouts, you will be required to have submitted this form acknowledging that you have read and understand the TRYOUT POLICY & PROCESS 2026-27 document in its entirety.

Each player participating in tryouts must complete this form.

Your player will not be given their pinnie unless this form has been received.

Acknowledgements

The person signing this form represents all parents/guardians of the player and this acknowledgment will be applied to same.

I understand that decisions about how many rep teams in each division are based on specific factors, not the desire of the parents in the division to have additional rep teams.
I understand that if a member of my family is verified to be spectating during Phase 1-3, my player will have their next ice time cancelled and a ranking of zero entered for that session
I understand that if a member of my family is verified to be communicating with coaches or evaluators during any phase of tryouts, my player will have their next ice time cancelled and a ranking of zero entered for that session
I understand that I must return my pinnies in good condition on the specifed Pinnie Return Day, otherwise I will be charged $25 per pinnie.
