Prior to arriving at your player's check-in for tryouts, you will be required to have submitted this form acknowledging that you have read and understand the TRYOUT POLICY & PROCESS 2025-26 document in its entirety.

Each player participating in tryouts must complete this form.

Your player will not be given their pinnie unless this has been received.
