4-day Masterclass- Workshop:

There will be two class levels. Please register to the level that fits your dancing experience.

Sunday Show:
Sunday, August 25th, 3:00 PM at the Atlas Performing Arts Center. Students must attend all of class on Friday, Saturday, and Sunday to participate in the show. Students should plan to wear any color leotard, dance clothing, or Guerrero style outfits. More details to come. Each student will have 1 complimentary ticket for a guest to attend the recital. Tickets will be available at the door for purchase. Ticket cost is $40.

Liability Waiver

CORAZÓN FOLKLÓRICO DC INC RELEASE, INDEMNIFICATION, AND WAIVER OF LIABILITY. By initialing this form, I do hereby agree to the following acknowledgment below. All participants and students must complete this form before participating in any dance classes, Masterclass, dance workshops or rehearsals at, BALLET DE MEXICO DE AMALIA V.B. HERNANDEZ. If participant is under age 18, a parent or guardian must also sign this form. Admission to class and/or rehearsal will not be granted if this form is not received prior to class and is not properly signed.
Waiver of Liability

I recognize and understand the risks of physical injury inherent to dance and dance training and I fully assume those risks. I hereby release BALLET DE MEXICO DE AMALIA V.B. HERNANDEZ its affiliates dance studios, event sponsors, employees and dance teachers from all liability for injuries sustained or illnesses contracted while attending or participating in any dance classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and hold harmless BALLET DE MEXICO DE AMALIA V.B. HERNANDEZ affiliate dance studios, event sponsors, employees and dance teachers for liabilities, costs and judgments arising from acts of omissions committed by me or my child which result in injury or damage to any person or property.

Protection of Property

I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances.

I hereby release BALLET DE MEXICO DE AMALIA V.B. HERNANDEZ, its affiliate dance studios, event sponsors, employees and dance teachers from all liability for loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, or performances. I also agree to abide by any rules, regulations and policies set forth by BALLET DE MEXICO DE AMALIA V.B HERNANDEZ

Medical Attention In case of physical injury or medical emergency

I hereby authorize BALLET DE MEXICO DE AMALIA V.B. HERNANDEZ to make necessary arrangements to transport myself or my child to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In extreme emergency, or if my child is under 18 years of age, I understand that BALLET DE MEXICO DE AMALIA V.B. HERNANDEZ will attempt to notify the person(s) I have named below as my emergency contact(s) of my condition.

Photo Release

BALLET DE MEXICO DE AMALIA V.B. HERNANDEZ reserves the right to use photographs and videos taken during classes, workshops, performances, or other affiliated events for the purposes of instruction, advertising and promoting BALLET DE MEXICO DE AMALIA V.B. HERNANDEZ and its programs. Students, or parents of students who are minors, who do not wish to comply with this policy must notify BALLET DE MEXICO DE AMALIA V.B. HERNANDEZ prior to participation in class. PROHIBITED TO VIDEO TAPE AND TO TAKE PICTURES DURING CLASSES AND PRESENTATION AS WELL LIVE SOCIAL AND IPHONE VIDEOS. Unless specified differently only by Elisa Fulks.

Acknowledgement of Waiver

In signing this Release, I acknowledge and represent that I have fully informed myself of the content of the waiver and hold harmless agreement by reading it before I sign it, and I understand that I sign this document as my own free act and deed; no oral representations, statements, or inducements, apart from the written statement, have been made. I further state that I am at least eighteen (18) years of age and am fully competent to sign this agreement; and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems which preclude or restrict my or my child’s participation in this activity, and that I will pay any medical costs that may be attendant as a result of injury to me or my child.
I, undersigned does hereby acknowledge that I am aware of the dangers and the risks to my person and property involved in participating in any event, show, performance, workshop, rehearsal, practice, meeting, or activity (collectively “events”) for, related to, and on behalf of Corazón Folklórico DC LLC (“Corazón”). I understand that these events involve certain risks for physical injury, including, but not limited to death, and injury to property. I also understand that there are potential risks of which may presently be unknown. Because of the dangers of participating in this activity, I agree to fully comply with the applicable laws, policies, rules and regulations, and any instructions regarding participation in this activity or the events. I understand that Corazón has no responsibility or liability for any injury resulting from any events. I acknowledge that I voluntarily elect to participate in these events with knowledge of the dangers involved, and hereby agrees to accept and assume any and all risks of property damage I may cause, and/or personal injury, including death. I also agree to release, discharge, waive, and hold harmless Corazón, its successors, assigns, officers, administrators, volunteers, employees, or agents, from any and all liability, claim, demands, suits, and/or causes of action that may indirectly or directly result from participation in these events for or on behalf of Corazón, including, but not limited to, any injury that may arise while I am on the premises for events. Waiver of Liability and Indemnification:In consideration for being allowed to participate in the above-referenced events, on behalf of myself, my personal representatives, heirs, next of kin, successors and assigns, I forever: a. waive, release, and discharge Corazón and its successors, assigns, agents, officers, volunteers, administrators, and employees from any and all liability for my death, disability, personal injury, property damages, property theft or claims of any nature which may hereafter accrue to me and/or my estate as a direct or indirect result of participation in these events; and b. agree to defend, indemnify, and hold harmless Corazón, its successors, assigns, agents, officers, volunteers, administrators, and employees, from and against any and all claims, demands, suits, causes of actions of any nature including all costs, expenses and attorneys’ fees, which in any manner result from my actions during these events. This release, indemnification, and waiver shall be construed broadly to provide a release, indemnification, and waiver to the maximum extent permissible under applicable law. I, the undersigned, affirm that I am freely signing this agreement. I have read this form and fully understand that by signing this form I am giving up legal rights and/or remedies which may otherwise be available to me regarding any losses that I may sustain as a result of participation in the activity or events. I agree that if any portion of this agreement is held invalid, the remainder will continue in full legal force and effect. I also agree that the laws of the District of Columbia shall govern, construe and enforce all of the rights and duties of the parties arising from or related in any way to the subject matter of this contract without regards to the application of any conflict of law rules.
