    I understand that ballet and dance conditioning are physical activities involving movement, stretching, balance, jumps, turns, strength training, flexibility exercises, and cardiovascular activity.

    I understand these activities involve inherent risks, including but not limited to:

    • muscle soreness
    • strains
    • sprains
    • falls
    • broken bones
    • joint injuries
    • concussion
    • other physical injuries


    I voluntarily choose to participate despite these risks.

    Health Statement

    I understand it is my responsibility to consult a physician if I have concerns about participation.

    I agree to stop participating immediately if I experience pain, dizziness, illness, or other concerning symptoms.

    Online Class Participation

    For virtual lessons, I understand that I am responsible for ensuring a safe practice environment, including:

    • adequate floor space
    • removing obstacles
    • appropriate footwear (if applicable)
    • proper flooring whenever possible, particularly when jumping or dancing en pointe
    • internet connection and technology


    I understand Hayden Creech Coaching cannot control my home environment and is not responsible for injuries resulting from unsafe practice conditions.

    Assumption of Risk

    I understand that no physical activity is completely free from risk.

    I knowingly and voluntarily assume all risks associated with participation in ballet instruction, dance conditioning, workshops, rehearsals, performances, private lessons, virtual classes, and related activities offered through Hayden Creech Coaching.

    Release of Liability

    To the fullest extent permitted by applicable law, I release and hold harmless Hayden Creech Coaching, Hayden Creech, assistants, contractors, facility owners, and affiliated instructors from claims arising from ordinary risks associated with participation in classes and activities.

    This release does not apply where liability cannot legally be waived, such as for gross negligence or intentional misconduct.

     

    Student Responsibilities

     

    Students agree to:

    • arrive prepared to participate

    • wear appropriate dance attire

    • follow instructor directions

    • respect other students (if applicable)

    • participate safely

    • communicate injuries or discomfort immediately

    Payment & Cancellation Policy

    • Payment is due before scheduling a lesson. If purchasing a bundle, payment is due before scheduling the first lesson.
    • Private lessons canceled with less than 24 hours' notice may be forfeited unless otherwise agreed.
    • Refunds are handled on a case-by-case basis.

  • Attendance & Scheduling
    Arrival:

    • Students are expected to enter the GoogleMeet waiting room 5 minutes before their scheduled class time. 
    • For in-person classes, students are expected to arrive at least 10 minutes before their scheduled class time and warmup in the hallway.
    • Classes will not be extended to accommodate late arrival. Students will be considered “no shows” and charged accordingly if they do not communicate within 15 minutes of the start of the scheduled class time.

    “No shows” and Cancellations:

    • “No shows” are charged for the full class time. Exceptions are made on a case-by-case basis.
    • Cancellations made with less than 24 hours notice will result in a $15 fee. Emergencies will be evaluated on a case-by-case basis.
    • For cancellations made with at least 24 hours notice, students may reschedule at their convenience via Calendly.
    • Should the instructor need to cancel, you will be notified as soon as possible and your class will be rescheduled.

    Payments & Expiration

    • Payment is due before scheduling a lesson. If purchasing a bundle, payment is due before scheduling the first lesson. 
    • Refunds are handled on a case-by-case basis.
    • Class bundles do not expire while Hayden Creech Coaching remains actively offering services. Scheduling is subject to instructor availability.
    • Gift cards expire after 1 year and may not be refunded.

    Dress Code & Class Preparation

    Appropriate dancewear:

    • While leotard and tights are not required, students should wear flexible, form-fitting clothing that allows the instructor to clearly observe alignment and movement. 
    • Bare feet are preferred for PBT classes to allow for safe execution of exercises and effective corrections. 
    • Ballet flats and/or pointe shoes are required for ballet classes. Remember to bring toe pads and spacers if applicable. Socks are discouraged.
    • It is your responsibility to bring well-maintained pointe shoes to your class. If your shoes are no longer safely supporting you, we may ask you to take them off. It is always best to bring another pair of pointe shoes with you.
    • Hair must be secured away from the face.

    Equipment:

    For PBT, we utilize many props. However, those without equipment are still encouraged to register. There is plenty we can do without props in the beginning. After learning about you and your goals, we will guide you on what prop might be most essential for your training.

    Pointe Expectations
    Pointe work requires appropriate preparation, strength, technique, and approval from a student's primary ballet teacher.

    Students participating in pointe work must:

    • Receive approval for pointe work from their primary ballet teacher.
    • Have pointe shoes properly fitted by a qualified fitter.
    • Maintain the strength, alignment, and technique necessary for safe pointe participation.
    • Bring well-maintained pointe shoes that provide appropriate support.
    • Students and guardians must disclose any changes to pointe eligibility, including injury, a recommendation to discontinue pointe work, or withdrawal of pointe approval from the student's primary ballet teacher.

    Hayden Creech Coaching reserves the right to modify or discontinue pointe exercises if the instructor determines that additional preparation or safety considerations are needed.


    Online Class Expectations
    Virtual ballet and dance conditioning classes are designed to provide instruction, feedback, and training in an online format. Because the instructor cannot physically assess or modify the student's environment, students and families are responsible for creating a safe space for participation.

    Before each online class, students agree to:

    • Prepare a clear, safe practice area free of obstacles, clutter, pets, and other hazards.
    • Ensure that the flooring and space are appropriate for the movements being performed. Students should avoid participating on surfaces that are slippery or unsafe for dance movement. 
    • Wear appropriate clothing and footwear for safe participation.
    • Have water available and take breaks as needed.
    • Position the camera so the instructor can provide the best possible visual feedback.
    • Test technology before class and notify the instructor of technical difficulties.


    For students under 18, parents or guardians are responsible for ensuring that the student has a safe practice environment and appropriate supervision based on the student's age, maturity, and training level.

    Hayden Creech Coaching provides instruction and guidance during virtual classes but cannot control the student's physical surroundings, equipment, flooring, or participation choices. Students and families remain responsible for making safe decisions during online training.

    Classroom Expectations & Safety

    Students are expected to treat instructors, classmates, facilities, and equipment with respect. Disruptive, unsafe, or disrespectful behavior may result in a parent/guardian contact or the student dismissal from that class. 

    Students are expected to:

    • Follow all instructor directions and safety recommendations.
    • Work within their own physical abilities and limitations.
    • Avoid attempting advanced skills, including jumps, turns, pointe work, or conditioning exercises beyond their current training level.
    • Communicate immediately if they experience pain, dizziness, discomfort, or an unsafe condition.


    Communication & Scheduling
    Email communication is preferred. We will communicate via text at your request and/or in the case of a last-minute cancellation. Once you have our phone number, you are welcome to communicate that way. Expect responses to occur within 24-48 hours.


    Scheduling:

    PBT scheduling is conducted via Calendly, which is linked on each page of our website. Select a time slot and fill out a short information sheet. You will receive a confirmation email immediately following registration and a reminder email the day before your scheduled class. GoogleMeet links are included in both emails.
    Online ballet classes and all in-person classes are scheduled via email or text.
