The only fee involved is a minimum donation of the agreed upon adoption fee to be paid in full for the dog stated in section III to DHRA (Doghouse Rescue Academy). This fee is to help cover the expenses associated with rescuing said dog in section I and is non-refundable. A copy of this adoption contract will be emailed to the Adopter stated in section II.
The dog stated in section I has been neutered or spayed. In adopting this dog from DHRA, the Adopter stated in section II agrees to keep him current on all medical care, vaccines, and abide by county and state laws. The Adopter in section II will hold Doghouse Rescue Academy Inc (DHRA) harmless and all representatives from any liability or harm or damage caused from said dog stated in section I. The Adopter stated in section II releases Doghouse Rescue Academy Inc (DHRA) and all the board members and Representatives from any liability in regards to said Rescue.
RETURN POLICY
If for any reason the adoption is not found to be satisfactory for the dog stated in section I or the adopting owner stated in section II, the DHRA requires and the Adopter stated in section II fully understands and agrees that the dog stated in section I must be returned to our program. The dog stated in section I must not be sold or otherwise disposed, but must be returned to a representative of the DHRA who is designated as the Rescue Coordinator, or who is delegated to stand in as a representative of the Rescue Coordinator by the Rescue Coordinator, on the DHRA website: www.doghouserescueacademy.org. The DHRA reserves the right to take back any rescue animal that is being neglected, not properly cared for, is being allowed to run free unsupervised, or if it is determined that it would be in the best interest of the dog stated in section I.
CARE
The adoptive family agrees to provide a loving home with comfortable and proper shelter, quality nutrition; and supplements as prescribed by a qualified and licensed vet, all necessary medical care, and agrees that their rescued animal will not be allowed to run free without supervision. Proper ownership requires that the adopted animal shall not be maintained outside as a yard animal. The animal must be housed indoors and provided with dry and comfortably heated or cooled facilities. Failure to provide proper ownership will require that the animal be surrendered to the DHRA. Representatives of the DHRA may conduct home visits to check on how the animal is doing and if everyone is doing well.
An important part of proper ownership includes socialization for the animal which includes regularly including the animal on family outings and taking the animal to safe places where the animal can meet new people of differing ages and other socialized animals. The point of this paragraph is that the DHRA strives to have its dogs to be fully incorporated into their adoptive family's lives; and to be part of new, stimulating, and safe experiences. Keeping the animal only at home and in the yard are not proper ownership of a DHRA animal.
Additionally, the adoptive family understands that the DHRA puts a lot of loving care and time into its animals and may request updates about how the animal is doing and photos of the animal and the adoptive family. The adoptive family agrees to provide such updates and photos as requested.
The adoptive family agrees to never leave the rescue animal in an unairconditioned car, to never leave the rescue animal unsupervised and tied out in public, to be aware if the animal is standing for too long (5-15 minutes depending on the temperature) in the sun without shade or water; and to remove the animal to a cooler, more comfortable environment, to keep the animal off of hot pavement, sand or other hot surfaces. To not allow the animal to drink water or be bathed from a hose that has been laying in the sun until the water has cooled to a safe drinking temperature, to be aware that heat exhaustion is a problem for animals, and to do everything possible to avoid too much heat for the animal.
The adoptive family agrees, as a part of proper ownership, to keep the rescue animal current on heartworm preventative.
LOSS
If the dog stated in section I is lost and/or stolen, the Adopter stated in section II shall notify the DHRA immediately, but the Adopter stated in section II shall be responsible for all expenses associated with the recovery of the lost animal. DHRA will provide guidance only.
INFORMATION ABOUT THE ADOPTED ANIMAL
The DHRA has furnished the Adopter stated in section II with information about the background of the adopted animal. The DHRA has no reason to believe this information is false; however, the DHRA cannot and does not represent that such information is accurate. The Adopter stated in section II may not and is not relying upon the accuracy of the information supplied by the DHRA in entering into this adoption agreement. Instead, the Adopter stated in section II has fully and freely examined the adopted animal and has based the decision to adopt upon its own evaluation of the adopted animal’s character and condition. The Adopter stated in section II is aware that the adoptive animal may have come with no knowledge of previous behavior or care. All known available veterinary records are being released to the Adopter stated in section II.
CONTACT INFORMATION
Should the Adopter stated in section II move or change contact information (telephone number and/or email address) while the rescue animal is in the adopter’s possession, the Adopter stated in section II agrees to contact the DHRA and advise them of the new address & contact information.
MARKETING AND MEDIA RELEASE
I as the Adopter stated in section II acknowledge that I am over 18 years old, and will grant permission to DHRA and all the board members and Representatives the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of me, or members of my family, for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any medium. I hereby release DHRA and its legal representatives for all claims and liability relating to said images or video. Furthermore, I grant permission to use my statements that were given during a verbal or written conversation, with or without my name, for the purpose of advertising and publicity without restriction. I waive my right to any compensation.
By my signature, I attest that I have read the TERMS OF ADOPTION and understand them and agree to them.
