SPAY/NEUTER CONTRACT
Permanent ownership of this dog is contingent upon compliance with this Agreement between Doghouse Rescue Academy (‘DHRA’) and the ‘Adopter’.
In consideration of an adoption donation as described in the adoption agreement, DHRA has transferred all rights, privileges, and responsibilities associated with the dog described in the Adoption Agreement.
The Adopter acknowledges that as of the date of this Agreement, the dog is not spayed or neutered, and the Adopter agrees to have the dog spayed or neutered before (s)he reaches the age of six (6) months.
The Adopter shall deposit, in addition to the adoption donation, a sterilization deposit of $100.00. The sterilization deposit is refundable to the Adopter if proof of spaying or neutering of the dog is presented to DHRA not more than thirty (30) days after the date of the spay/neuter surgery. The deposit shall be returned to the adopter no more than fourteen (14) days after the receipt of the proof of spay/neuter.
If the Adopter fails to provide timely proof of spaying within the time set forth above, the deposit is forfeited to DHRA. This Agreement will be enforced and the appropriate legal authority will be notified. In addition, all rights and ownership of the dog shall automatically terminate and transfer back to DHRA, and DHRA will immediately assume possession of the dog. The Adopter shall pay all reasonable attorney’s fees incurred in attempts to repossess the dog, as well as $500.00 in damages. If proof of spay/neuter is provided after the time limits set forth above, the deposit shall remain forfeited and the ownership of the dog shall be determined on a case-by-case basis.
DHRA may allow extensions of the time periods upon receipt of a certificate from a veterinarian licensed to practice veterinary medicine in the United States that certifies that the dog has a medical condition which prevents him or her from being spayed/neutered, or that it would otherwise be detrimental to the health of the dog to be spayed/neutered. In such a case, the dog shall be spayed or neutered when the dog is healthy enough, as certified by a veterinarian licensed to practice veterinary medicine in the United States. The dog shall be spayed or neutered within thirty (30) business days of that certification, or the conditions set forth above will be enacted.
Proof of spay/neuter (scheduled appointment confirmation, a spay/neuter certificate, and receipt) should be sent in the form of a scanned document or photo to adopt@doghouserescueacademy.org, please contact Carey at (570) 460-6890 or Dawn at (239) 848-5992 to inform us as soon as possible.
WHEN TO SPAY/NEUTER: The spay or neuter should be performed by the indicated date in section I with proof of appointment and receipt for the surgery emailed to adopt@doghouserescueacademy.org unless specified otherwise by the rescue.
WHY TO SPAY/NEUTER: Fixing your dog prevents pregnancy and the birth of unwanted puppies. Our goal in rescue is to eliminate the need for our services by enforcing spay/neuter and not contributing to the overpopulation of dogs that leads to the euthanasia of so many every day. In addition to the overpopulation issue, it also has proven health benefits for dogs of both sexes. For both males and females, being altered has been proven to result in an increased life span. Wouldn’t you like as much time as possible with your pet? For males, neutering also eliminates the risk of testicular cancer, along with other health benefits. For female dogs, spaying eliminates uterine infections and significantly decreases the risk of mammary tumors.
WHERE TO SPAY/NEUTER: Spay/neuter is available through Associates in Veterinary Medicine and Surgery at 12377 S Cleveland Ave Suite 6 Fort Myers FL 33907 that is included with your adoption fee. If you choose to go to a different veterinarian practice, you will be responsible for paying for the surgery and other associated costs at your own expense.
If you fail to spay/neuter the dog by the agreed upon date stated above, this will result in DHRA claiming the animal back into our possession immediately; your adoption fee will not be refunded and you will not be allowed to adopt any animal from our rescue in the future.
TRAINING CONTRACT
When adopting a dog from DHRA, we require proof of enrollment in a training class by an approved trainer and/or facility before you pick up you are in possession of the animal. If a dog requires specific training classes, our DHRA trainer will specify the training need(s) in Section II
