PLEASE NOTE:

Submitting this form does not guarantee approval; all requests will be reviewed by the Transportation Department, and you will be notified once a decision has been made.

If you are currently on the waitlist for an AM or PM bus stop, you do not need to complete this form unless you would like to make a change to your original request.

I acknowledge and understand that Transportation is a privilege, and I have reviewed and agree to Dayspring Academy's Transportation Policies and Rules, as published on the transportation page of the Dayspring Academy website.

By signing your signature below, you are requesting consideration for bus transportation and agree to the Transportation Parent/Learner Agreement information:

1) Parents are advised to have the learner at the bus stop 10 minutes prior to the scheduled morning pick up time.

2) A parent/guardian, 6th grade sibling or older, or designated party must be at the designated afternoon bus stop in order for an elementary learner to be let off the bus. Please note: the above includes the Calvary Chapel Worship Center stop. The parent/guardian should arrive 10 minutes prior the estimated stop time. If the parent/guardian is not at the bus stop, the learner will be returned to Dayspring Harmony campus ACT II program at the end of the bus schedule. The parent/guardian will be required to pick the learner up at Dayspring Harmony campus ACT II building.

3) The learner MUST remain seated at all times, must follow ALL bus rules, and listen to the driver at ALL times.

4) The Dayspring school buses are an extension of the school day and learners should behave accordingly.
