Assumption of Risk, Waiver, and Release of Liability
Spring 2026
RISK FACTORS:
I understand and acknowledge that participation in dance classes, rehearsals, practices, and/or performances involves some inherent risks such as, but not limited to, personal injury, property damage, loss, illness, or death. Furthermore, participation in these activities includes possible exposure to and illness from infectious diseases, including but not limited to MRSA, influenza, and COVID-19.
ASSUMPTION OF THE RISKS AND RELEASE:
I recognize that there are certain inherent risks associated with the above-described activities and I assume full responsibility for personal injury, property damage, loss, illness, or death to myself and my family members, and further release and discharge EDMC and IDC and their employees for injury, loss or damage arising out of my or my family’s use of or presence upon the facilities of EDMC, whether caused by the actions of myself or my family, EDMC, IDC or third parties.
DAMAGES DUE TO NEGLIGENCE:
I agree to pay for all damages to the facilities of EDMC caused by any negligent, reckless, or willful actions by me or my family.
MEDICAL AUTHORIZATION:
In the event of an injury to the named participant below, while participating in the Activities provided or sponsored by IDC and/or EDMC, I give permission to IDC and/or EDMC, its employees, representatives and/or agents of IDC and/or EDMC to arrange for all necessary medical treatment for which I shall be financially responsible. This temporary authority will begin on the date this form is submitted and will remain in effect for the duration of active enrollment with EDMC or June 24, 2026, whichever occurs first.
IDC and/or EDMC shall have the following powers: 1) The power to seek appropriate medical treatment or attention on behalf of myself or my child as may be required by the circumstances, including without limitation, that of a licensed medical physician and/or a hospital. 2) The power to authorize medical treatment or medical procedures in an emergency situation. 3) The power to make appropriate decisions regarding clothing, bodily nourishment, and shelter.
INJURIES
IDC and EDMC have a strict policy to prevent dancers from aggravating injuries by participating in dance against the advice of their doctor. Dancers who sustain injuries, whether dance-related or not, that require a doctor's attention, must present a signed note from their doctor indicating what limitations in movement the dancer may have while healing from the injury and the expected date to resume full physical activity. The dancer must present a signed note from their doctor indicating when they are cleared for dance. These types of injuries include, but are not limited to:
- Broken bones
- Severe sprains/strains
- Concussions
- Dislocated joints
- Stitches
Dancers who come to class with visible signs of injury (such as casts, splints, braces, crutches, wrapped joints, etc.) who have not communicated about the injury or presented a doctor's note will be asked to observe class until such information is obtained.
This policy does not apply to dancers who regularly employ the use of athletic tape or other compression braces and supports for wrist, knee or ankle joints. However, if your dancer does regularly use these support devices, please let their teachers know that it is not an injury.
I, the undersigned Impact Dance Company (IDC) at Expressions Dance & Movement Center (EDMC) participant, affirm that I have read this entire Assumption of Risk and Waiver of Liability for EDMC and IDC for the Spring 2026 Impact Dance Company Season.
