I understand that massage therapy provided by BodyIgnite, LLC and its practitioners, is intended for relaxation and therapeutic purposes only.

I acknowledge the following:
1. Health History
  I have disclosed all relevant health information and understand that it is my responsibility to keep the therapist informed of any changes in my health or physical condition.
2. Consent to Treatment
  I voluntarily consent to receive massage therapy. I understand that I may communicate any discomfort during the session and that the therapist may adjust techniques accordingly.
3. Scope of Practice
  I understand that massage therapists do not diagnose illnesses or injuries, nor do they prescribe medical treatment. Massage therapy is not a substitute for medical care.
4. Liability Waiver
  I release and hold harmless BodyIgnite, LLC, its practitioners, and affiliates from any liability or claims resulting from injury or discomfort during or after the massage session, except in cases of gross negligence or misconduct.
5. Cancellation Policy
  I have read and understand BodyIgnite’s 24-hour cancellation policy. I agree to pay any applicable fees for late cancellations or missed appointments.
