  • I understand that Radiant Day, LLC is a care navigation and provider directory service. Radiant Day is not a healthcare provider and does not provide clinical care, diagnosis, or treatment.

    By signing below, I consent to the following:

    1. I authorize Radiant Day to review the information I provided in my intake form for the purpose of matching me with vetted healthcare providers, therapists, doulas, or other support professionals in the Radiant Day directory.

    2. I consent to Radiant Day sharing my name, email address, phone number, and the general concerns I described (for example, "postpartum anxiety" or "IEP advocacy") with up to three matched providers so those providers may contact me directly to schedule services.

    3. I understand that any healthcare or clinical services will be provided by the matched provider under their own policies, consent processes, and professional license - not by Radiant Day.

    4. I understand that Radiant Day uses Jotform Gold, a HIPAA-compliant platform, to collect and store my information securely. I have read the Radiant Day Privacy Policy at theradiantday.com/privacy-policy.

    5. I may revoke this consent at any time by emailing hello@theradiantday.com.

    6. If I am signing on behalf of someone else, such as my minor child or a person for whom I am the legal representative, I confirm that I have the legal authority to sign for them.

    Revocation will not affect any actions already taken based on this consent.
