ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY, AND
INDEMNITY AGREEMENT

Legynds Sports Training LLC

DECLARATIONS
 
This Agreement is entered into between Legynds Sports Training LLC (“Trainer”)
and the undersigned (“Client”). The provision of personal training or athletic
performance services by Trainer to Client and Client’s participation in any related
activities, use of any premises, facilities, or equipment are expressly conditioned
upon the execution of this Agreement.

ASSUMPTION OF RISK
 
By signing this Agreement, you acknowledge and agree that if you engage in any
physical exercise, training, or athletic activity, whether on-site, off-site, or at any
location where Legynds Sports Training LLC services are provided you do so at your
own risk. You assume full responsibility for any and al injuries, damages, or losses you may sustain, whether arising from your participation in exercise or other related
activities, or while using any facilities or equipment provided or arranged by Trainer.
This includes but is not limited to the use of any training equipment, participation in
field or gym-based activities, or movement through any facility area. You understand
that injuries or damages may occur from known and unknown risks, including the
negligence (active or passive) of Legynds Sports Training LLC, its owners, coaches,
employees, agents, contractors, or representatives. You voluntarily participate with
the ful understanding of these risks and assume al associated responsibility for any
injury, ilness, loss, or damage to yourself or your property.

RELEASE OF LIABILITY
 
In consideration of receiving training or related services from Legynds Sports Training
LLC, you, on behalf of yourself and your personal representatives, heirs, executors,
administrators, agents, and assigns, hereby release and discharge Legynds Sports
Training LLC, its owners, affiliates, employees, contractors, agents, and
representatives from any and al claims, causes of action, or liabilities (known or
unknown) resulting from negligence, whether active or passive. This release includes,
but is not limited to, any claim resulting from malfunction or failure of any training or
exercise equipment, improper maintenance, negligent instruction or supervision,
negligent hiring or retention of staff, or slips, trips, or falls on any premises where
training occurs.

INDEMNIFICATION
 
You agree to indemnify, defend, and hold harmless Legynds Sports Training LLC, its
owners, affiliates, and representatives from any loss, liability, damage, or cost
(including reasonable attorney’s fees) that may result from your participation in
training or use of any facilities, equipment, or services, except where caused by gross
negligence or willful misconduct.

ACKNOWLEDGMENTS
 
You expressly agree that this Agreement is intended to be as broad and inclusive as
permitted by the laws of the State of Arizona, and that if any portion is held invalid, the remainder shal continue in ful force and effect. You acknowledge that Legynds Sports Training LLC provides athletic performance and fitness training services and is not a manufacturer or seler of fitness equipment. This release is not intended to waive claims of gross negligence or intentional acts.

ENTIRE AGREEMENT & UNDERSTANDING
 
You acknowledge that you have carefully read this Assumption of Risk, Waiver and
Release of Liability, and Indemnity Agreement, fully understand its terms, and
voluntarily sign it with full knowledge of its significance. You understand that by signing this document, you are waiving certain legal rights, including the right to bring a lawsuit or assert a claim against Legynds Sports Training LLC for ordinary negligence. No oral statements or representations apart from this written agreement have been made.
