Flight Booking Form

For domestic flights, please ensure the name matches the government-issued ID/Passport. For international flights, a passport is required for travel. If you do not have a passport at the time of request, we will discuss timeframes needed to obtain your passport before booking your flight. The traveler will assume all risks with passport arrivals as most flights are non refundable unless the premium ticket options are available for a refund (travel credits may also be available). The Diamond Travel Fee is nonrefundable in the event of cancellation, late arrival of passports, etc.
This form is completed for FLIGHT ONLY REQUEST. Once your form has been received, Diamond Travel Group will confirm receipt and send details on where to send your payment. Payment is due before flights are booked.

We can assist with group flights (10+ people departing/returning from the same city).

Rates:
- $25 for domestic flights (per person, paid at the time of booking);
- $50 for international and group flights (per person, paid with contract rates).
  - Deposit of $100 is due before group flight research begins; deposit is applied to total fee balance due.
