  • This form is to prepare the Liturgy for when our Bishop presides over the Confirmation Mass.

    Please submit this form at least three weeks before the celebration. All fields with asterisks* must be filled out. Click the Submit button when the form is complete or click the Save & Continue button to finish the form at a later time. Please do not copy the form and send it.

  • Gloria is always sung on Sunday (except In Advent & Lent), Solemnities, Feasts, and Ritual Mass even in Advent & Lent.

  • Presidential Prayers (Collect,Offertory, After Communion) from Ritual Confirmation or from Sunday*
  • If choosing the presidential prayers from Ritual Confirmation (refer to Roman Missal page 1184) which Preface of the Holy Spirit that will use?

  • Readings: Please check the diocesan website www.fwdioc.org/sacrament-of-confirmation  for selections of first and second readings if other than Sunday. 

  • Day of Mass
  • Homily by Bishop or Other*

  • During the Ritual of Anointing

  • Chrism Oil will be provided by (be sure to have bread & lemon for washing after anointing)*

  • Music, either instrumental or singing in the low voice, during the confirmand receive the anointing. Be sure to have ample music selection depending on the number of confirmand.

  • Preparation for Ritual: Coordinator of Confirmation needs to rehearse with candidates so they know the proper responses during the ritual. Please provide a name tag for each candidate with first name followed by Saint name to assist Bishop during the anointing.
