This form is used to apply for diocesan approval to become an academy and join one of our partner Multi Academy Trusts.

Before completing this form, you should have submitted Form A1: Registering Interest in Joining a Multi Academy Trust and received confirmation from the Diocese that you may proceed.

The Diocesan Board of Education (DBE) has a statutory role in approving any change of status for Church schools. In Leeds, all applications are considered through a formal diocesan process.

By submitting this form, you confirm that you are willing to provide any additional information needed to support your application.

Your application will be considered by the DBE School Organisation Subcommittee. We will confirm the timing of this once your completed form has been received. You may be requested to attend an online meeting with board members, alongside the MAT, to present your reasons for joining before permission is granted. 

Please note that parts of this form must be completed by your chosen partner MAT and will need to be signed by the CEO. Please use the SAVE function to generate a link you can share with them.
