It is a requirement of employment for all staff who will be undertaking diving operations either recreationally (for fun) or professionally (Instructors and Dive Masters) with us to complete an Annual Dive Medical.

If you are under 18 your medical form must be signed off by a Parent or Guardian.

If you have not completed a form in the last 12 months please download and review the Dive Medical requirements on the relevant form from our WHS Page.
I have reviewed the Dive Medical requirements and I am not aware of any reason that I could not complete the required Dive Medical
