Pre-Employment Transition (Pre-ETS) Form
Thanks to our diverse funding and collaborative partnerships, including with the Rehabilitation Services Administration/Vocational Rehabilitation (RSA/VR), youth can participate in our programs at no charge. To receive this funding and facilitate Diverse Ability Incorporated's programs, participants must complete a Pre‑Employment Transition Services (Pre‑ETS) Form. This Form is the document RSA/VR uses to authorize funding support for eligible students.

Tips for Completing the Pre-ETS Form

Page 1: Must be completed by you and your legal guardian or parent (if applicable).

Page 2: Must be completed by a professional who can verify that you are a student who has a disability. A professional may be any of the following:
- Teacher
- School Counselor
- Principal
- Disability Resource Center
- Vocational Rehabilitation Counselor
- A Medical Practitioner, such as a doctor or therapist.
If you are unable to obtain verification from a professional:
- Complete and upload Page 1.
- Upload a copy of your IEP, 504 plan, SSI/SSDI award letter, or a medical diagnosis document that states that you are a person who has a disability.
- Upload proof of school enrollment.
If you are a minor or not your own guardian, your parent or guardian must assist and co-sign these documents with you. Even as a legal adult, you may benefit from support in completing these documents.

If you need assistance completing the Pre-ETS Form, you may use the Pre-Employment Transition Services Request Form Checklist provided by the Rehabilitation Services Administration/Vocational Rehabilitation Services to keep track of your progress. You can also email us or call 602-425-5135.
Tips for Submitting Pre‑ETS Forms
- The pre‑filled Pre‑ETS form is provided to the student’s parent or legal guardian to complete. You may also include the RSA Pre‑ETS Form Checklist as a helpful resource.
- Once completed, the student returns the form to the school/program for verification.
- School/program personnel use this system to upload completed Pre‑ETS forms for their students.

To complete your Pre-ETS Form:

- Complete the Pre-ETS Form, then click the download icon (an arrow pointing down toward a horizontal line or a box.)

- A box to save your document should appear.
- Enter your name into the "Save As" field.

Please note that additional documentation may be required when submitting the Pre-ETS form. Examples include:
- Individualized Education Plan (IEP) or 504 Plan (only page(s) which outline diagnosis);
- Medical or psychological documentation with diagnosis by a licensed professional;
- Proof of acceptance/enrollment into a post-secondary educational program; and
- Proof of legal guardianship if the student has a legal guardian.
