🔹 How it Works:
1. Complete this intake form, which gathers essential information about your background, experiences, and the relevant legal requirements and circumstances that have led you to seek assistance.
2. After completing this form, Dr. Long will personally review your responses and assess the relevant factors in your case.
3. You will receive a free written case summary via email outlining key insights about your case.
4. After reviewing the case summary, you can decide whether to proceed with the full evaluation.
Next Steps After Your No-Cost Case Review
If you decide to move forward after receiving your case summary, the next steps will be:
✅ Scheduling the evaluation appointment – To secure Dr. Long’s time, a deposit payment of $1,100 is required, which applies toward the total evaluation cost of $2,200. The evaluation process typically takes 2-3 weeks after the appointment. Expedited processing is available for an additional $1,000.
✅ Attending the virtual evaluation with Dr. Long – USCIS requires that all evaluations include an interpreter. It is your responsibility to provide an interpreter who is fluent in both English and your native language. Your interpreter must either be physically present with you or have access to the video evaluation via a strong internet connection. A link will be provided for them to join the session.
If you need an interpreter, we can provide one for an additional $200.

Confidentiality: The information you provide may be included in the final report submitted to USCIS or other relevant parties. Dr. Long will ensure that only relevant and necessary details are included to protect your privacy whenever possible.
Accuracy Matters: It is crucial to provide honest and thorough answers. Any discrepancies or omissions may impact the evaluation's findings and your legal case.
How to Complete the Form
Time Required: Set aside 1–2 hours to complete the form.
Device: Use a laptop or desktop computer for ease of completion, though any device with internet access will work.
Saving Progress: You can use the 'Save' feature at the bottom of the form to pause and resume later.
Answering Questions: Provide complete and accurate answers. If a question does not apply to you, write “N/A” instead of leaving it blank.
Structure of the Form
