- The cost is $25.00 per scholar.
- The group will depart at 9:30 AM and should return at 1:30 PM.
*DIA will not accept verbal approval*

I have read the information above and give my permission for my scholar to attend the enrichment field trip listed above. I understand that my child will be traveling to and from by bus. I also permit my child to receive the necessary medical treatment(s) in case of emergency
