1st Grade Field Trip Permission Slip: Southern Belle Farms

Please fill this form out before your scholar attends the trip.
A field trip is scheduled for our Kindergartners to vist Southern Belle Farms located at 1659 Turner Church Rd, McDonough, Ga 30252. Please keep the follwoing in mind:
- Trip Date: Wednesday, September 30th
- Trip Cost: $25.00 (per scholar)
- The group will depart at 9:00 AM and is estimated to return at 1:00 PM.
*DIA will not accept verbal approval*

**This form, and trip payment, must be completed by the parent/guardian. Payment can be made in Infinite Campus**

I understand that my child will be traveling to and from by bus. I also authorize, in case of an emergency, necessary medical treatment(s).
