I am responsible for providing complete and accurate information for the preparation of my tax return. I affirm that all information provided, including this questionnaire and any supporting documents, is true, correct, and complete to the best of my knowledge.
Documentation Requirements
I agree to maintain and provide, upon request, all necessary documentation (including receipts, records, and supporting statements) required to substantiate income, deductions, and credits reported on my tax return.
Reliance on Client Information
Empresa Solutions will rely on the information I provide and will not independently verify its accuracy or completeness unless otherwise agreed in writing.
Review Responsibility
I understand that I am responsible for reviewing my completed tax return prior to signing and filing. I agree to promptly notify Empresa Solutions of any errors, omissions, or changes.
Tax Law & Audit Disclosure
I understand that tax laws are subject to interpretation and change. While Empresa Solutions will use professional judgment in preparing my return, no guarantees are made regarding the outcome of any tax position. I acknowledge that my return may be subject to examination by taxing authorities and agree to provide additional information if requested.
Additional Services & Fees
Services beyond tax return preparation—including audit representation, amended returns, tax planning, bookkeeping, or additional consultations—may incur additional fees.
Privacy & Data Security
Empresa Solutions will take reasonable measures to safeguard my personal information but cannot guarantee absolute security of electronic communications.
Authorization
By signing below, I authorize Empresa Solutions to prepare my tax return based on the information provided. I understand that my return will not be electronically filed until I review it and sign the applicable e-file authorization forms.
