      If you have already toured the school and completed the family interview, you will complete this enrollment form. This initiates the formal admissions process.
      1. Church Affiliation - FaithWay Christian Academy exists primarily for the edification of the faithful families of FaithWay Baptist Church and other churches who agree with our statement of faith. We desire that FaithWay Christian Academy students and families maintain a unity of mind and spirit regarding doctrine, philosophy, and standards for worship.
      2. Faithful Church Attendance - The parent’s should demonstrate their love for the Lord, commitment to spiritual growth, and desire to train their children by faithfully attending FaithWay Baptist Church or a ministry that agrees with our statement of faith. FaithWay Christian Academy admits and retains only students who, along with their parents, faithfully attend their local church services.
      3. Pastoral Reference - FaithWay Christian Academy requires a pastoral reference as part of the enrollment process.
      4. Transferring in Good Standing - FaithWay Christian Academy only admits a student who transfers from the former school in good standing. The school is not designed to meet the special needs of students with unusual behavior, spiritual, academic, social, or physical problems. In evaluating a potential student, admission testing and interviews are customarily required.
      5. Willingness to Meet Enrollment Expectations - At FaithWay Christian Academy we seek to set and maintain standards for student conduct, dress, philosophy of life, procedures, and scholarship that honor the Lord and demonstrate a spirit of excellence. FaithWay Christian Academy admits only those students who want to be enrolled. In addition, students and parents must willingly sign the Statement of Faith and Standards of Conduct agreement. Students must continue to indicate this desire via the annual re-enrollment process.
      6. Five years old by Aug. 1 (Kindergarten) - Kindergarten applicants must be five years old by August 1.

    • PLEASE USE THIS FORM

      1. The student's financial account must be current at the time of re-enrollment.
      2. The student's financial account must remain current to the end of the school year. If the account is current at the time of re-enrollment but becomes past due after re-enrollment, the student's name will be removed from the re-enrollment list until the account is brought current.
      3. The Admissions Committee reviews re-enrollment applications. They determine which students may not be re-enrolled and notify the parents as needed. Reasons for being denied include but are not limited to:
        1. Lack of regular church attendance
        2. Excessive marker and demerit levels
        3. Failure to abide by the Code of Conduct
        4. Pattern of late payments along with failure to communicate in advance with the Finance Office
        5. A pattern of excessive absences and late arrivals
        6. Lack of parental partnership
        7. Learning difficulties which necessitate additional support that FCA is not prepared to offer
    • NON-DISCRIMINATION POLICY: FaithWay Christian Academy admits students of any race, color, national and ethnic origin to all the rights, privileges, programs, and activities generally accorded or made available to students at the school. It does not discriminate on the basis of race, color, national and ethnic origin in administration of its educational policies, admissions policies, scholarship and loan programs, and athletic and other school-administered programs.

  • It is FaithWay Christian Academy's policy that all parents and guardians with legal custody sign this form. If the legal and/or physical custody of the child is defined by court order or decree, the order or decree, with all current modifications, must be uploaded to this form. Any additional modifications to the custody of the child must be relayed to the FCA promptly, accompanied by copies of such modifications.

  • FACTS SIS Transition

    This JotForm captures your initial rnrollment information. As FCA transitions to FACTS SIS as our new student information system, all remaining required documentation including medical forms, statement of faith, emergency contacts, and handbook acknowledgment will be completed directly in FACTS SIS. Families can expect access to FACTS SIS and further instructions by the end of May 2026.
