LIABILITY WAIVER

I, the undersigned parent or legal guardian of the above-named minor, acknowledge and agree to the following terms as a condition of my child’s participation in the Fundamental Sports Group basketball program:

Assumption of Risk – I understand that participation in basketball involves inherent risks of injury, including but not limited to, sprains, fractures, and concussions. I voluntarily assume all such risks associated with my child’s participation.

Release of Liability – I hereby release, waive, and dischargeFundamental Sports Group, its coaches, staff, volunteers, and affiliated organizations from any and all liability, claims, or demands arising from my child's participation in practices, games, travel, or related activities.

Medical Authorization – In the event of an emergency, I authorize team representatives to obtain medical treatment for my child. I assume full financial responsibility for any medical expenses incurred as a result of participation.

Insurance Responsibility – I understand that Fundamental Sports Group does not provide medical or accident insurance, and it is my responsibility to ensure that my child is covered under an appropriate insurance policy.

SOCIAL MEDIA WAIVER

I, the undersigned parent or legal guardian, grant permission to Fundamental Sports Group to capture and use photographs, videos, and other media of my child for promotional purposes, including but not limited to:
- Social media platforms (Facebook, Instagram, Twitter, etc.)
- Team website and newsletters
- Promotional materials (flyers, posters, etc.)
- Local media coverage
I understand that:

My child’s image may be used without compensation.
The team will not disclose personal information (such as full name or contact details) without further consent.
