Performer Agreement and Media ReleaseBy completing and submitting this registration form for "Exhale: Testimonials and Talents" (the "Event"), you (referred to as "Performer" or "Participant") understand, acknowledge, and agree to the following legally binding terms set forth by FTD Ministry ("the Organization"):1. Ownership of Recorded MaterialsThe Performer acknowledges and agrees that any and all audio, video, photographic, or digital recordings made during the Event (collectively, the "Recorded Materials") are the sole and exclusive property of FTD Ministry. FTD Ministry holds all rights, title, and interest in the Recorded Materials, including all copyrights.2. Media Release and Discretionary UseThe Performer hereby grants FTD Ministry a perpetual, irrevocable, worldwide, royalty-free license to use, reproduce, distribute, display, perform, edit, and modify all performer information, bios, likenesses, voice recordings, performance videos, and submitted images. FTD Ministry may use these materials at its absolute discretion for any purpose, including but not limited to promotional materials, social media, website content, marketing, future broadcasting, and ministry outreach, without further notice, review, or financial compensation to the Performer.3. Waiver of LiabilityThe Performer releases and holds harmless FTD Ministry, its staff, volunteers, directors, and affiliates from any and all claims, liabilities, demands, or causes of action arising out of or connected with the Performer’s participation in the Event or the subsequent broadcasting and distribution of the Recorded Materials.4. Acknowledgment and AcceptanceBy checking the "I Agree" box (or submitting this form), you certify that you are at least 18 years of age (or have a parent/legal guardian's consent), that you have read and fully understand this agreement, and that you freely consent to all of its terms.
