2025 Cotillion Application Package

APPLICATION PACKAGE

Participants must complete the online application, upload their official school transcript, which depicts a 2.5 GPA or better, and pay the non-refundable registration fee to complete the registration. The non-refundable registration fee is $450 for Debutantes and $300.00 for Jr. Escorts. The Jr. Escort fee includes the cost of their tuxedo rental. All Debutantes must have a Jr. Escort to participate. The Foundation is not responsible for providing Jr. Escorts. Registration will close on June 17, 2025. Debutantes not fully registered will not be permitted to continue with the Debutante season. Full registration means all documents are uploaded, and Debutante and Jr. Escort fees paid in full.

My daughter has permission to participate in the Forever Pink Foundation, Inc. Debutante Cotillion. Moreover, I permit her image and likeness to be captured in photos and videos for marketing and advertisement of the Cotillion. I understand that participation is voluntary, and my child may refuse to participate at any time. I also acknowledge that the participation fee is non-refundable and will not be returned regardless of my daughter’s participation. I further acknowledge that for my daughter to be presented at the Debutante Cotillion, she must meet all presentation requirements.

Forever Pink Foundation reserves the right in its sole discretion to discontinue the presentation and participation of any participant. All monies remitted to the Foundation in support of the fundraiser are nonrefundable and tax exempt.
