Fox Hoops LLC Payment, Participation Agreement & Code of Conduct
I agree that I am taking a spot in a Fox Hoops program. I understand that there are no refunds. In the event my child cannot participate due to an injury, sickness, or any other conflict, a pro-rated credit will be issued. The credit may be used towards a future Fox Hoops program and does not expire.
I agree that Fox Hoops may utilize photographs, videos, testimonials, email addresses, and information relating to my child's participation in Fox Hoops activities on the Fox Hoops website, social media platforms, and marketing materials.
I hereby authorize the agents of Fox Hoops LLC to act for me according to their best judgment in any emergency requiring medical attention. I hereby release, discharge, and indemnify Fox Hoops LLC, its staff, affiliated entities, officers, agents, and employees from and against any and all claims, liability, causes of action, lawsuits, or awards arising out of or in connection with my or my child's participation in the program.
I agree that program locations and times are subject to change based on availability, inclement weather, and enrollment.
Fox Hoops Code of Conduct
Players are expected to respect their teammates, coaches, referees, and opponents at all times. They should work hard, give their best effort, and maintain a positive attitude during practices and games. Players must arrive on time and come prepared with the proper gear and uniform. Good sportsmanship is required on and off the court. Parents and guardians are expected to support all players and encourage them positively from the sidelines. They should respect the coaches, referees, and other families and allow the coaches to coach without interference. Any absences or concerns should be communicated directly and respectfully with the coaching staff. Fox Hoops has a strict zero-tolerance policy for fighting, bullying, harassment, profanity, or any disrespectful behavior from players, parents, or coaches. Violations may result in suspension or removal from the program.
I have read the Fox Hoops Code of Conduct with my child and promise to abide by it. I authorize this form to be used and updated accordingly for all future Fox Hoops programs my child participates in.
