This form allows you to recieve a 6 digit alphanumeric code unique to you that can be used to simplify registering for future events. Once this form is completed, you simply need to input that code into the form for each event you wish to register for.

- Handmade Items: Items or products you specifically make
- Direct Sales: (also called catalog sales) You sell items for a company from a catalog
- Other Sales: Your items are not handmade and are not a catalog sales company
Business: You are not selling physical items, but rather advertising or gathering information for prospective clients or appointments

Food/Beverage: Selling foods and/or beverages

Junior Entreprenuer: Business Owner is under 18
