Client Intake form - pet sitting

Thank you for choosing us to care for your pet when you are away!

Please answer the questions below so we can have the correct and up to date information to care for your pet appropriately.
We send picture updates via text or email daily. You will always know when we have left your pet for the day, how they are doing and if there any concerns.

Please note :
-If we are doing day visits and your pet has free reign of the house, we are not liable for anything that is broken (vases, furniture, etc) prior or after our visit.
-We will not crate any dogs that will be left alone for more than 4 hours unless you sign below releasing us from any liability for injury to your dog.
-We will not visit dog parks with your dog.
-If your animal bites any employee, understand under Colorado Animal Control policy, they may have to be quarantined.
