GENERAL LOCAL FIELD TRIP AUTHORIZATION

I,    First Name   Last Name     give FUN KIDS CLUB CALGARY INC., and the employees (if applicable), permission to take my child,   First Name   Last Name on short field trips, nature & Community walk and other outings as part of the Extended Kindergarten and Out of School Care Programs.

SUPERVISION PLAN:

• Proper ratios shall be applied as per Child Care Licensing Regulations at minimum.  Where 7 or more children, whether on or off the program premises, a minimum of 2 adults, one of whom is a primary staff member, are on duty, and Children are, at all times, under supervision that is adequate to ensure their safety, well-being and development for any and all scheduled field trips.
• To ensure children’s safety both indoors and outdoors through adequate, active, and effective supervision
• Directing and closely monitoring children when carrying out activities that may involve some risk, such as playing near water, near doorways, or during transition times when Children may gather in large groups.
• Observing play and anticipating what may happen next in order to provide caregivers with the opportunity to assist children and intervene in the event of potential danger.

Transportation: Walking / On Foot
