1. I have registered my child, * * , for Summer Camp at the Greater Denton Arts Council (“GDAC”). I, the undersigned parent/legal guardian of * * , authorize said child’s participation in the Greater Denton Arts Council 2026 Dance & Movement Camp (“GDAC Dance & Movement Camp”), including all related activities. I fully understand all of the dangers, hazards, and risks that are associated with and may occur as a result of my child’s participation in the GDAC Dance & Movement Camp and related activities. I understand that these dangers and risks may result in property damage, sickness, and/or physical injury.
2. In consideration of my child being permitted to participate in the GDAC Dance & Movement Camp, I agree to assume full responsibility for all risks. I further agree to release, waive, and covenant not to sue GDAC, as well as employees and any students acting as employees of GDAC (referred to collectively as "Releasees"), from and against any and all liability, claims, demands, actions, causes of action, suits in equity, whatsoever arising out of or related to any loss, damage, or injury. I further agree to indemnify the Releasees from liability, claims, demands, actions, causes of action, or suits in equity arising out of loss, damage, or injury that occurs as a result of my child’s negligent or intentional act or omission while participating in the GDAC Dance & Movement Camp.
3. Emergency Medical Care: In the event of illness, injury, or accident involving my child while participating in the GDAC Dance & Movement Camp, I authorize the Releasees to act on my behalf in obtaining appropriate medical care. This authorization includes approval for any necessary non-emergency or emergency medical treatment. In the event of a medical emergency, I understand that GDAC staff will make reasonable efforts to contact me or the listed emergency contact as soon as practicable.
4. I understand and agree that Releasees are granted permission to authorize temporary first aid treatment, if necessary, for my child and that such action by Releasees shall be subject to the terms of this Release, Waiver, Indemnification, and Agreement not to Sue. I understand that Releasees assume no responsibility for any injury to my child or for any related cost which might arise out of or in connection with such authorized first aid treatment.
5. Property Responsibility: I understand that GDAC is not responsible for any damage or loss of personal possessions while at camp or while participating in any camp activity. I also understand that campers are expected to respect the facility, equipment, artwork, and property of GDAC and others. If my child breaks, damages, or defaces any property as a result of their direct or indirect actions, I agree to accept financial responsibility for the repair or replacement of the damaged property.
6. I understand that GDAC Dance & Movement Camp embraces messy play. I understand that clothing worn to the camp may get stained, and will dress my child in clothing that is meant to get messy. I acknowledge that GDAC is not responsible for any clothing that is stained or ruined while attending class.
7. Camper Conduct and Safety Policy: To ensure a safe, respectful, and positive environment for all campers and staff, participants are expected to follow camp rules and treat others with kindness and respect. GDAC maintains a zero-tolerance policy for unsafe or harmful behavior, including but not limited to kicking, biting, hitting, or other acts of physical aggression toward campers, staff, or property.
If a camper engages in behavior that is deemed unsafe, harmful, or significantly disruptive to the camp environment, the Camp Director will contact the camper’s parent or guardian immediately. Such behavior may result in the camper’s immediate removal from the GDAC Dance & Movement Camp without refund.
The Camp Director will make reasonable efforts to work with families to support positive participation; however, the safety and well-being of all campers and staff is our highest priority, and GDAC reserves the right to dismiss a camper at any time if their behavior poses a risk to others or interferes with the safe operation of the camp.
8. Pick-Up Policy: I understand that campers must be picked up from camp by a parent/guardian who is listed as the emergency contact, or another adult listed as an authorized pick-up person on the camp registration form. For the safety of all campers, authorized adults may be required to present a valid photo ID at pick-up. GDAC staff reserve the right to verify identification before releasing a camper.
I understand that campers must be picked up promptly at the end of their scheduled camp session. A $15 late pick-up fee will be charged beginning at 3:15 PM (or 15 minutes after the scheduled end of the camp session). An additional $1 per minute will be charged for each minute thereafter until the camper is picked up.
9. I understand that GDAC Dance & Movement Camp must meet a minimum number of participants for each session to remain financially viable; therefore, if a particular session doesn’t reach this level, classes may be canceled. If, for any reason, classes are canceled, GDAC will provide a full refund to all registered participants.
10. It is my express intent that this Release, Waiver, Indemnification, and Agreement not to Sue shall bind myself, my child, the other members of my family and spouse, if I am alive, and my estate, family, heirs, administrators, personal representatives, or assigns, if I am deceased.
11. In signing this Release, Waiver, Indemnification and Agreement not to Sue, I acknowledge and represent that I have carefully read the document and understand its contents and that I sign as my own free act and deed. I further state that I am at least eighteen (18) years of age and fully competent to sign; and that I have executed this Release for full, adequate, and complete consideration fully intending to be bound by the same.
