  • By agreeing below, I understand that this booking request form does not guarantee me an appointment. GOLDWRKS will review your inquiry and make sure He is the right fit for your request. If denied, it is not personal. I understand that this is a non refundable deposit. If my submission has been denied, my deposit will be returned. If I need to reschedule, this deposit can be transferred over. If I need to reschedule due to any reason, GOLDWRKS requires that I let him know at least 72 hours ahead otherwise my deposit will be forfeited. I understand that if I make changes to my design after submitting this request and booking my appointment the cost may vary from the quote I was given.
