This form is to be submitted ONLY for designs from the sheets below. All designs start at $150 and are between 1-3 inches. Pricing is for arms and legs; torso, neck, head and face will have a +$50 added on.

2 for $250 (save $50)

In an attempt to speed up my booking process, please make sure to fill out the information correctly. Be ready to submit a $100 deposit. I do charge by the piece; my minimum is $200, and flash/custom typically runs in the 350-500 range. If your budget falls below that I'm more then happy to talk to you about creating something within your budget.

By agreeing below, I understand that this booking request form does not guarantee me an appointment. GOLDWRKS will review your inquiry and make sure He is the right fit for your request. If denied, it is not personal. I understand that this is a non refundable deposit. If my submission has been denied, my deposit will be returned. If I need to reschedule, this deposit can be transferred over. If I need to reschedule due to any reason, GOLDWRKS requires that I let him know at least 72 hours ahead otherwise my deposit will be forfeited.
