Please complete this form by 11:59pm on May 25, 2026 to register your swimmers.  Payment ($11 per swimmer) will be due on June 1 at the board meeting (7 pm at Battlefield Green).  For those swimmers that you add after the May 25th registration, payment will be due at Champs registration.

If you have any graduating seniors (graduating in May/June 2026) who have turned 19 on or before June 1st but intend on swimming this season, please note them below. Otherwise, leave that part of the form blank.

An adjustment needs to be made in the system so they are eligible to swim; therefore, we must have their names listed below.  Do NOT CHANGE Birthdays

REGISTRATION VALIDATION: By listing my name below, I authorize that all swimmers listed on our Roster are "members in good standing" with our Club/Association. They have full use of the swimming facilities of the club, and have paid all applicable dues and fees. They also presently comply with all rules and regulations of the member club, association, or organization, and are currently listed as bona fide members of said club.
