This form is for Girl Scout members only whose families are John Ball Zoo Members and want to register for Girl Scout Day at the Zoo.

Since you do not need to purchase tickets through the main registration, this $2 charge per Girl Scout ensures they receive an event patch.

Completion of this form prior to March 31st will guarantee your Zoo Day patch and/or the Badge Activity booklet included with your badge purchase.
