Initial* I am the custodial parent and responsible financially to Gymnastics Unlimited(I may bill a 3rd party, but I understand it is NOT the responsibility of Gymnastics Unlimited to collect from any person other than the signee.

   Initial*   Participation is entirely his or her own choice and with the understanding of risk of accidental injury involved in any activity involving unusual motion or height.

If a minor, the parents acknowledge that they know of this injury risk the minor is assuming. 

Initial*   In the event of an emergency requiring medical attention, I hereby grant permission to the personnel at Gymnastics Unlimited to administer first aid, emergency personnel and/or medical personnel designated by the Gymnastics Unlimited to attend my son/daughter (or ward).

Initial* Participant assumes full responsibility for all injuries and damages which may occur in or about the premises and he or she does hereby fully and forever release and discharge the gym operator, all associated gyms, their owners, employees, and agents from any and all claims, demands, damages, rights of action present or future, resulting from or arising out of the participants use of the gym and/or its facilities.

Initial* By enrolling my child in the Orchard Park Gymsters Team Program, I understand that a yearly, non-refundable registration fee is due each year and tuition will be paid through ACH withdrawal on the 1st of each month.

Early bird registration is available to current team members before becoming available to new team members.

• Initial*   If I choose not to have automatic withdrawals, tuition will be paid prior to the 15th of the previous month.  

• Initial*   I understand that a late fee of $10 will be charged on the 16th if not paid.

• Initial*   I also understand if the 15th is on a day the gym is closed, I understand tuition should be paid PRIOR to the 15th.

Initial* Tuition is based on the number of practices from September-June divided into 10 equal payments. 

This takes into account meet weekends, gym closures due to holidays and 3 “emergency” closures that the team practice may be canceled.

• Initial*I understand this is a year round sport.

• Initial*If withdrawing is necessary, there must be a two week written notice and all financial responsibilities must be fulfilled (monies due to Gymnastics Unlimited & Booster Club fees).

Initial*Gymnastics Unlimited reserves the right to remove my child from class for non payment.

Initial*I understand that policy may change without notice and Gymnastics Unlimited will post policy changes via email.

Initial*I understand it is my responsibility to provide a working email as this is Gymnastics Unlimited's main communication with parents.

It is my responsibility to keep abreast of Gymnastics Unlimited's communications.

Initial*I, nor my child, will not enter Gymnastics Unlimited if myself or my child has or had(in the past 24 hours):

1. a fever of 100.4 above
2. any contagious virus
3. has not been symptom free of any contagious virus for at least 24 hours.

Initial*In addition, I understand, even with precautions taken by Gymnastics Unlimited, becoming ill is still a risk.

Initial*I am aware that individual and group publicity photos and videos are taken from time to time and in consideration for my child’s participation I hereby grant permission for my child’s likeness to be used in Gymnastics Unlimited publicity and/or advertising.
                
Initial*   It is also understood that no compensation will be given by Gymnastics Unlimited for use of pictures or videos.  No names will be used.

Initial*Gymnasts will be monitored by a staff member if they are being picked up by a vehicle; otherwise parents/guardians should park in a spot in the lot and come into the facility to pick up their child.

• Initial*The circle is for drop off and pick up ONLY.

• Initial*Parking in the circle is prohibited.

• Initial*I understand that IF I need to get out of my car, I will PARK it in a spot in the lot.

Initial*I understand Gymnastics Unlimited does NOT offer make ups for missed practices.

Initial*I understand that canceled practices due to holidays, meets and weather are taken into consideration when the tuition is calculated and will not need to be “made up.”

Initial*Gymnastics Unlimited does NOT refund paid tuitions.

Credits MAY be offered depending on circumstances.

Initial*Team fee is based on the number of gymnasts and although it may be broken down into payments for convenience, it must be paid in full regardless if my child competes or remains in the program or not.

Initial*Meet entry fees will be paid prior to deadline, even if the deadline is not my child’s practice day.

If paid after deadline I will incur a late fee and/or possible forfeiture of my child’s entry into the competition.

Initial*I am required to volunteer at the gymnastics meets hosted by the Orchard Park Gymsters

Initial*I will partake in all “general” fund raisers that benefit the whole team.

Initial*My child, being part of a team program, will be attending all competitions unless otherwise given prior permission to miss.

Initial*This is not an all inclusive list of the Orchard Park Gymsters Booster Club Policies.

• (Initial)*I have read this release form regarding any and all classes at Gymnastics Unlimited.

• (Initial)*   I agree to follow all and understand that there is an assumption of risk involved with participating in the sport of gymnastics.

• (Initial)*   I agree not to hold Gymnastics Unlimited, its officers, agents or instructors responsible for any damages, injuries permanent disabilities or even death which may occur with my association to Gymnastics Unlimited.

• (Initial)*   I will give proper notice of any injury that may limit participation in any class.

• (Initial)*   I understand and give permission for treatment to be initiated by Gymnastics Unlimited.

• (Initial)*   I understand policies may change without prior but notice will be given via email, website post and newsletters.

• (Initial)*   It is my responsibility to keep updated Unlimited communications.
