(Initial)* Participation is entirely his or her own choice and with the understanding of risk of accidental injury involved in any activity involving unusual motion or height.

  (Initial)*   If a minor, the parents acknowledge that they know of this injury risk the minor is assuming.

(Initial)*   In the event of an emergency requiring medical attention, I hereby grant permission to the personnel at Gymnastics Unlimited to administer first aid or other medical personnel designated by the Gymnastics Unlimited to attend my son/daughter (or ward).

He or she does hereby fully and forever release and discharge the gym operator, all associated gyms, their owners, employees, and agents from any and all claims, demands, damages, rights of action, present or future, resulting from or arising out of the participants use of the gym and/or its facilities.

Tuition Policies Statement
(Initial)* Monthly tuition is due by the 15^th of the previous month.

For example: April is due BY March 15^th.

Payments received in the office after the 15th will be considered late.

If the 15th is a day the office is closed, payment is required PRIOR to the 15th to avoid being late.
Payments can be made by either:
- calling the gym during business hours
- stopping at the desk
- paying through the website
- We also offer autopay upon email request.
(Initial)* I understand that a $10 late fee will be added on the 16th of each month if payment is not received by the 15th.

(Initial)* I understand that I pay for my CHILD'S SPOT in his/her class, NOT by their attendance and there is NO pro-rating due to lack of attendance.

(Initial)* Gymnastics Unlimited reserves the right to remove my child from class for continuously late and/or non-payment.

Dropping from Classes
(Initial)*Gymnastics Unlimited must have a 2-week written notice in order for you to not be responsible for tuition.

(Initial)*Non-attendance does not assume withdrawal!!

(Initial)* I understand that I am responsible for tuition accrued if I do not give a 2-week written notice.
(Initial)*   To drop from a class or from the program, a WRITTEN 2 WEEK NOTICE of cancellation must be given to the office. The drop notice can be found on the website.

(Initial)*   I will be responsible for the portion of the tuition which is covered by the two-week notice.  This is calculated from the date that the office receives the notice in person or via email.

(Initial)*  Telling staff in person or over the phone is not notice.
(Initial)* The written 2-week notice does not pertain to the summer program; which is paid in full up front.

Summer is not refundable.

Paid tuition claims a spot in the 8-week program and is therefore not refundable due to the briefness of the session

(Initial)* Gymnastics Unlimited does not refund paid tuitions.

Additional Health and Facility Policies
(Initial)* I understand that policy may change without notice and Gymnastics Unlimited will post policy changes via email, on the website and in the monthly newsletter.
(Initial)* Please do not talk to the students while they are in class. It can be very disruptive. Please do not call students out of their class. If you need your child, please go to the desk and let them know.
(Initial)* Parents and siblings are not allowed out in the gym area.
(Initial)* I/We understand it is my responsibility to provide a working email as this is Gymnastics Unlimited’s main communication with parents.

(Initial)* It is my responsibility to keep abreast of
Gymnastics Unlimited communications.
(Initial)* I/We, nor my child, will not enter Gymnastics Unlimited if myself or my child has or had:
1. a fever of 100.4 or above
2. a cold
3. diarrhea, pink eye, step., vomiting or any other contagious virus
4. not been symptom free of any of the above for at least 24 hours
(Initial)* In addition, I/We understand, even with precautions taken by Gymnastics Unlimited, becoming ill is still a risk

Authorization for use of photos in advertising purposes
(Initial)* I am aware that individual and group publicity
photos and videos are taken from time to time and in consideration for my child’s participation I hereby grant permission for my child’s likeness to be used in Gymnastics Unlimited publicity and/or advertising.

(Initial)* It is also understood that no compensation will be given by Gymnastics Unlimited for use of pictures or videos.

DROP OFF/PICK UP POLICY
(Initial)*   I understand that I may drive through circle to leave my child off to a staff member at the door.

(Initial)*   If I need to get out of my car for any reason, I will PARK in the lot and walk my child into the building.

(Initial)*   The same applies at pick up.
(Initial)* If I choose to park my car in the lot at drop off or pick up, I must walk my child into and from the building

(Initial)* I understand that the circle is for drop off & pick up ONLY.

(Initial)*Parking in the circle is prohibited.

(Initial)* I will not drop my children off early or pick them up late.

CANCELLED AND MISSED CLASSES
(Initial)* I understand Gymnastics Unlimited does NOT offer make ups for missed classes.

(Initial)* Gymnastics Unlimited WILL offer make up classes for PLANNED closings. (ex.: Thanksgiving, Memorial Day, etc.)
(Initial)*I understand Gymnastics Unlimited offers make up classes during school breaks for ONLY scheduled closings.

At times they will be offered PRIOR to the class actually being missed.

It is my responsibility to have my child attend those make ups.
CLASS CHANGES OR CLOSURES

(Initial)* I understand the classes may change or cancel due to weather or coach illness.

(Initial)* If classes are canceled, I understand that Gymnastics Unlimited will do their best to find a time to schedule a makeup.
